About The Position

This position will support the Jefferson City, MO region. Under the direction of Regional Director, the Clinical Liaison is responsible for the continued growth of census in assigned Assisted Living and Residential Care Communities. Requirements

  • Current RN or LPN license in good standing
  • Minimum of 2 years nursing experience preferred
  • Experience in skilled nursing, rehabilitation, hospice, home health, case management, or hospital discharge planning preferred
  • Excellent communication and relationship-building skills
  • Strong assessment and critical thinking abilities
  • Ability to multitask and work independently in a fast-paced environment
  • Proficient with electronic medical records and Microsoft Office
  • Valid driver’s license and reliable transportation
  • Must be a supportive team member, contribute to and be an example of teamwork
  • Ability to travel throughout Mid-Missouri often and willing to travel immediately if an unexpected need should arise in a facility
  • Ability to make independent decisions when circumstances warrant such action
  • Ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel and the public
  • Must have patience, tact, and willingness to deal with difficult residents, family, and staff
  • Must be able to manage sensitive information with a high level of confidentiality.

Responsibilities

  • Serve as the primary clinical and admissions liaison between hospitals, physicians, case managers, discharge planners, and the facility
  • Evaluate patient referrals for clinical appropriateness and level of care
  • Coordinate and facilitate admissions and transitions of care
  • Build and maintain strong relationships with referral sources and healthcare partners
  • Educate patients and families on services, care options, and admission processes
  • Collaborate with interdisciplinary teams to ensure continuity of care
  • Conduct onsite hospital visits and community outreach as needed
  • Maintain accurate documentation and timely communication regarding referrals and admissions
  • Participate in marketing and census development initiatives
  • Ensure compliance with all state, federal, and company policies and regulations
