About The Position

New England Life Care (NELC) is seeking a Clinical Liaison / Teach Nurse for a Per Diem weekend position at Brigham and Women's Hospital in Boston, MA. This role involves providing clinical assessments for patients referred for home infusion therapy, developing strategies to capture referrals, and building relationships with hospital staff for care coordination. NELC is a growing non-profit home infusion provider serving multiple hospital systems and is committed to patient-focused care and fostering a supportive work environment.

Requirements

  • Must have an LPN license in good standing in the state(s) where providing care / services.
  • Associate degree or diploma in Nursing
  • Two (2) years intravenous therapy experience in hospital or homecare.
  • Excellent patient assessment skills.
  • Valid driver’s license.
  • Excellent driving record.

Nice To Haves

  • Current BLS Certification preferred.
  • Knowledge of HIPAA Privacy and Security Regulations preferred.
  • Knowledge of JCAHO accreditation process and overall JCAHO standards preferred.
  • Working / practical knowledge of home infusion therapy administration / equipment use preferred.

Responsibilities

  • Provides clinical assessment of patients referred by the member hospitals for home infusion therapy to determine their capabilities to safely receive services at home with NELC.
  • Responsible for developing and implementing strategies to capture all referrals from the member hospital’s system of care (e.g. inpatient, ambulatory, etc.).
  • Responsible for relationship building with hospital staff for quality coordination of care between hospital and home.
  • Gathers and communicates demographic, clinical, and reimbursement information on patients referred to NELC to the intake department.
  • Develops relationships and seeks opportunities to bring all potential home infusion referrals to NELC, including within the hospital, among ambulatory care providers, and within the member hospital’s care providers.
  • Communicates results of insurance verification to hospital discharge planner and/or patient, and assists the reimbursement department in exploring all possible sources of reimbursement.
  • Assesses needs and abilities of the patient and caregivers, participates in developing a patient-specific training plan and plan of care, and participates in patient training when appropriate.
  • Facilitates the discharge or referral process by relaying orders and documentation necessary for discharge or referral to the appropriate NELC departments.
  • Acts as a clinical liaison between NELC and all appropriate departments at the hospital or the hospital’s system of care, including medical staff, care coordination/case management, social services, pharmacy, and nursing departments.
  • Monitors active and pending NELC patient status while hospitalized and provides ongoing communication regarding patient status to appropriate NELC staff.
  • Maintains a patient caseload averaging between 15 and 20 patients.
  • Comprehensively documents all interactions with patients and members of the healthcare team in the patient record and/or electronic progress notes.
  • Effectively communicates patient requirements and changes in patient status to appropriate members of NELC infusion and reimbursement teams, as well as Home Health Agency (HHA) staff.
  • Participates in daily patient care meetings and case conferences as required.
  • Participates as a member of the hospital’s care management team in hospital rounds to identify and actively manage patients who may be referred for infusion, and provides advice and recommendations to the hospital care team.
  • Accurate completion and reimbursement determination of therapy-specific reimbursement checklists.
  • Assists in the development and implementation of new programs and educational seminars to enhance NELC’s ability to meet community healthcare needs.
  • Works in conjunction with Partnership Development staff to identify and capture new business.
