About The Position

All Heart Home Care is seeking a Clinical Liaison / Business Development professional to serve as the face of the agency in the Middle Tennessee market. This full-time role focuses on building referral relationships within hospitals, physician practices, SNFs, rehab facilities, and ALFs, connecting patients to the care they need. The position requires an active clinical license (RN, PT, OT, or ST) and experience in business development, community outreach, or liaison work within the home health, hospice, or post-acute care sectors. The ideal candidate will have established relationships in the Middle Tennessee healthcare community, be comfortable working independently, and be driven by outcomes. All Heart Home Care is a family-owned, Medicare-certified home health agency with over 30 years of experience, known for its skilled nursing, therapy services, and specialized care.

Requirements

• Active clinical license required: RN, PT, OT, or ST
• Experience in business development, community outreach, or liaison work in home health, hospice, or post-acute care
• Established relationships in the Middle Tennessee healthcare community
• Comfortable working independently and managing your own territory
• Driven by outcomes. For patients and for the agency.
• Bring the clinical credibility, the market knowledge, and the relationship-building instincts.

Responsibilities

• Cultivate and grow relationships with discharge planners, case managers, social workers, and physicians
• Educate referral sources on our clinical capabilities, payer mix, and service depth
• Open new accounts and deepen existing ones across the market
• Track activity and referral growth in CRM
• Collaborate with our clinical team to ensure seamless patient transitions
• Represent All Heart at community and professional events
