About The Position

The Patient Transfer Coordinator expedites patient transfers from outside hospitals, urgent/immediate care centers, and physician practices for direct admission to the facility. This role involves coordinating with physicians to maintain an optimized referral within SGHS, collaborating with physicians, nurses, and other medical professionals to ensure timely coordination of transfers, and assisting with the transfer of patients to higher or lower levels of care. The coordinator serves as the contact person for bed management regarding direct admission concerns, ensuring unit throughput and compliance with the transfer process. They develop collaborative relationships with physicians to enhance quality practice and maintain sound intradepartmental and interdepartmental working relationships. The role requires acting to resolve issues in a timely manner, investigating unusual occurrences, and following appropriate reporting procedures. As a department team member, they support Transfer Center and hospital goals, prioritizing and performing multiple tasks effectively in a time-sensitive environment to achieve optimal outcomes. This includes coordinating and collaborating with physicians, hospital nursing units, transportation, bed control, environmental services, and the House Manager regarding patient placement. The coordinator communicates new admission information to clinical unit nursing staff and develops a plan for acceptance within 30 minutes. During high call volume or limited bed capacity, they remain calm and provide effective solutions, demonstrating a strong ability to identify, analyze, and solve problems. The role also involves modeling professional growth and development through participation in educational programs, identifying needs and setting goals for personal growth, meeting educational requirements, and assimilating knowledge into practice. They develop mechanisms to share new knowledge with team members and seek improvements aligned with departmental and organizational strategy. Other duties as assigned are also part of the role.
