About The Position

Join Saint Luke's Home Health and Hospice in a NEW ROLE!! We are seeking an RN to join us as a Clinical Intake Coordinator! This is a Full-Time, Monday-Friday, 9 am-5:30 pm hybrid role, allowing for remote work 2-5 days per week. The position involves alternating weekday and weekend shifts every 4-6 weeks, with a holiday rotation. An RN license for both Missouri and Kansas is required. Prior Home Health or Hospice experience is preferred, as is experience with referral acceptance and appeals. The Clinical Intake Coordinator facilitates the home health and hospice referral process by receiving referrals from multiple sources within and outside the St. Luke's Health System. This role performs an initial evaluation of referrals for home health appropriateness under Medicare, Medicaid, and commercial insurance regulations, determines the need for additional information from referral sources, communicates agency acceptance or decline of referrals, and processes accepted referrals. This role also provides specialized support for the Patient Accounting and Finance departments, serves as a liaison between the patient, facility/physician, and third-party payer, and prepares and defends the level of care and medical necessity for assigned cases. Collaboration with the physician advisor, payor representative, and site case managers is key to facilitating appropriate level of care decisions and billing status, ensuring compliance with Utilization Review standards and regulations. The role occasionally covers to ensure compliance with home health face-to-face requirements and county reporting for hospice.

Requirements

  • Registered Nurse (MO) Multi-State - National Council of State Boards of Nursing (NURSYS)
  • Associate Degree - Nursing
  • 2 years Applicable Experience
  • RN MO and KS required

Nice To Haves

  • Home Health or Hospice experience preferred
  • Experience with referral acceptance and appeals preferred
