About The Position

The Dietitian works as part of an interdisciplinary team to provide group and individual educational services related to nutrition and wellness. The Dietitian works collaboratively with providers and other health care teams to develop and deliver programs and services tailored to the individual or group needs. Serves as expert and resource within the organization in the use of food and nutrition to promote health and manage diseases. This dietitian would see both inpatients and outpatients. They would oversee the LTC and acute care operations in both Aurora and Virginia.

Requirements

East Market: Upon completion of 90 days of employment must apply for Advanced Practitioner Credentials through the Medical Executive Committee
Registered or registry-eligible with the Commission on Dietetic Registration of the American Dietetic Association within state(s) of employment.
Must be registered within state of employment within six months of hire or transfer.

Responsibilities

Identifies and assesses patient nutritional needs and creates care plans to address any identified risks.
Works with critical care patients in the ICU, writing nutrition-related orders.
Provides appropriate nutrition education.
Assesses wound-related nutrition needs and completes timely and thorough documentation in the electronic medical record.
Assesses for and diagnoses malnutrition.
Educates on diet according to disease state, medical condition, and health risk.
Coordinates discharge plans for patients returning home with nutritional support needs.
Writes orders for home nutrition support equipment.
Creates and educates patient on nutrition support plans.
Navigates insurance plans for coverage of home nutrition support.
Coordinates needs with the selected home medical supply company of patient’s choice.
Acts as consultants to rural Essentia Health site providers for high acuity nutrition care concerns.
