The Clinical Infusion Pharmacist IV delivers pharmaceutical care by providing patient-centered clinical service, medication information, education, medication preparation, and distribution to ensure safe, effective, and cost-efficient medication therapy. This role is responsible for ensuring safety in the compounding and dispensing of chemotherapy, biotherapy, and other infusion drugs. The pharmacist will provide direct patient care, patient education, and actively participate in clinical decision-making. They will also support policy and program implementation to ensure the safety of staff and patients during the preparation, administration, and monitoring of chemotherapy. Adherence to all local, state, and federal regulations, codes, policies, and procedures is required to ensure privacy and safety while delivering optimal patient care. The position also involves meeting ASHP requirements to precept pharmacy residents in an accredited residency program and participating in educational activities such as precepting pharmacy students/residents, providing in-services, or presentations to pharmacy, nursing, or providers.
