HOPE ~ HEALTH ~ HEALING Seeking a Clinical Housing Specialist to join our interdisciplinary team working with individuals diagnosed with a Serious Mental Illness (SMI) or General Mental Health diagnosis under Court Ordered Treatment at our South Mountain Recovery Health Center.

Requirements

  • GED/high school diploma with a minimum of 4 years of behavioral health employment experience, or An associates degree in the field of behavioral health and 2 years of behavioral health employment experience, or A bachelor's degree (not in the field of behavioral health) and 1 year of behavioral health employment experience, or A bachelor's degree in the field of behavioral health, or A master's degree or higher in the field of behavioral health.
  • Proficient in Microsoft Word, Excel, PowerPoint, & Outlook.
  • Must be able to pass typing assessment with minimum 25wpm.
  • Knowledge in use of Electronic Health Records - NextGen is a plus.
  • Excellent oral and written communication skills.
  • Must have valid Arizona driver’s license, be 21 years of age with minimum 4 years driving experience and meet requirements of Terros Health’s driving policy.
  • Must have a valid Arizona Fingerprint Clearance card or apply for an Arizona fingerprint clearance card (Level 1) within 7 working days of assuming role.
  • Must pass a TB Test.

Nice To Haves

  • Bilingual in Spanish/ASL is desired.

Responsibilities

  • Under supervision of site management, the Clinical Housing Specialist performs duties in support of therapeutic services and provides comprehensive direct support services which are structured to meet the needs of an adult population composed of SMI, dual diagnosed and/or referred persons.
  • Responsible for working with the member to facilitate access to housing opportunities, address housing permanency and support the independent living in a community setting; performs related duties as required.
  • Assess client needs and provide tailored housing resources.
  • Accompanies members on occasion to housing briefings and/or to explore housing options.
  • Tracks applications and referrals to housing programs and services for members.
  • Maintains accurate and up-to-date data demographics related to housing and homeless status.
  • Collaborate with community organizations to connect clients with social services that enhance their stability.
  • Utilize motivational interviewing techniques to empower clients in their housing journey.
  • Stay informed about changes in housing policies and advocate for client rights within the community.
