About The Position

To develop, review, collaborate on discipline specific training content and process improvement. To use their in-depth knowledge and expertise in primary care to deliver accurate and up-to-date information, advice, and solutions.

Requirements

  • Registered Professional Nurse, with current Michigan license, and Masters of Science in Nursing with a clinical nursing focus and nurse practitioner license.
  • Minimum 3 years experience working as a Provider at PACE with consistent illustration of foundational knowledge of PACE Model.
  • Minimum 5 years experience as a advanced provider in a health care setting.
  • Demonstrated knowledge and skill in evaluating, planning, implementing, and monitoring patient-centered approaches to care.
  • Knowledge of and compliance with applicable local, state, and federal laws and regulations.
  • Cultural competence and an ability to care for a diverse population.
  • Ability to learn and use electronic health record system.
  • Proficient in Microsoft Word, Excel, Outlook, PowerPoint.
  • Positive, flexible, and solution-focused attitude with ability to collaborate effectively in a team setting.

Responsibilities

  • Providing expert knowledge, training and insights into the nurse practitioner/provider role including: assessments, medication management, complex medical care, care planning and interventions.
  • Assisting in the development and review of project documentation, training content, and manuals
  • Developing and conduct on-boarding and competencies for providers.
  • Creating in partnership job aids and training regarding documentation in the EMR
  • Working with various teams to ensure accuracy and relevance of content
  • Identifying potential areas for process improvement and suggesting changes
  • Conducting thorough research to stay updated on changes in their field of expertise
  • Contributing to the development of policies and procedures relevant to their area of expertise
  • Participating in or leading training sessions, webinars, and workshops
  • Reviewing content developed by others for accuracy and completeness
  • Answering specific queries and providing guidance based on their expert knowledge
  • Consulting across business operations, providing mentorship, and contributing specialized knowledge.
  • Participate in determining technical or software needs
  • Resource and/or champion for discipline specific projects
  • Partners with the center leadership team to ensure effective operations and clinical activities.
  • Performs other related duties, as assigned by leader(s)
  • Ability to travel to all PACE SEMI centers, as this may be required for environmental audits, training, or completing competencies of primary care.
