The Clinical Registered Dietitian II is a member of the interdisciplinary health care team who provides general clinical nutrition care to all patients on units in which they are trained and competent to perform. The Clinical RD II identifies patients with malnutrition, increased protein and/or energy needs or other medical problems affecting nutrition status. Primary responsibilities include developing and implementing medical nutrition therapy using established standards of care and practice policies/guidelines; evaluating and monitoring the effectiveness and outcomes of the plan of care; ensuring continuity of care throughout the provision of care process; providing nutrition education to patient and family members on behaviors to accomplish patient goals; conducting educational programs for hospital staff and the community, as needed. Participate in performance improvement projects as assigned. In addition, for Clinical Registered Dietitian II working in the outpatient setting, provide nutrition counseling to those with specific medical diagnoses, altered nutrition status and those wishing to promote optimal health for chronic disease management. Delivers education through individual counseling, group classes, and community presentations using in-person and/or remote based tools as assigned.
