About The Position

The Diabetes Management Centres (DMCs) throughout Nova Scotia Health fall under the Primary Health Care portfolio. The DMCs work with primary care providers to provide patients with the support they need to manage their diabetes. The goal of the DMCs are to provide education and self-management support for patients to manage their blood sugars, cholesterol levels, and blood pressure. The Centres assist patients to maintain their health and prevent the complications of diabetes in a model that follows a mix of group, individual and virtual programming. The DMC model advocates and supports an interdisciplinary and integrated approach to diabetes education and management to provide patient centred care. Reporting to the Health Service Manager, the Clinical Dietitian (Diabetes Education) provides extensive disease management support and education to persons who are living with or at risk of developing diabetes. Key components of the role include but are not limited to disease management/care coordination; education; counseling, and program development. The Clinical Dietitian (Diabetes Education) participates in medication management ; insulin initiation; insulin adjustment and follow up; diabetes nutrition counseling; self-management support; physical activity counseling; and providing/facilitating links with resources in the community. The core of this position is providing patients with the skills and confidence to self-manage their chronic condition. This position provides coverage to Twin Oaks Memorial Hospital, Eastern Shore Memorial Hospital and Musquodoboit Harbour Memorial Hospital.

Requirements

  • Successful completion of a Bachelor of Science degree in Human Nutrition
  • Successful completion of a dietetic internship program accredited by Dietitians of Canada
  • Registration with Nova Scotia Dietetic Association (NSDA) is required
  • Willing to complete requirements to enact associated care directives to adjust insulin
  • Minimum of 3 years’ experience in profession
  • Registration with relevant health discipline licensing body
  • Certified Diabetes Educator, or able to obtain within 3 years of being employed in a Diabetes Management Centre
  • Knowledge in areas of Diabetes Care
  • Training and knowledge in motivational interviewing and behavior change techniques
  • Experience in delivering diabetes education to clients in one-on-one and group settings
  • Superior organizational, time management and problem-solving skills
  • Ability to work independently and collaboratively as part of an interdisciplinary team
  • Experience in adult education and demonstrated knowledge of adult education principles
  • Strong interpersonal and communication skills
  • Demonstrated Interest in applying new technologies and strategies to education
  • Travel to other sites is required; must have access to a reliable vehicle
