About The Position

Join Iowa Home Care as a Clinical Data Specialist, where you'll play a vital role in supporting quality patient care by ensuring accuracy and compliance within our electronic medical records (EMR) system. This behind-the-scenes position is essential to maintaining high standards of care through precise data management, seamless coordination with clinical teams, and diligent follow-up with providers. In addition to office responsibilities, this position also serves as a backup Home Health Aide (HHA) when needed to ensure continuity of care for our clients. Previous Home Health Aide (HHA) or Certified Nursing Assistant (CNA) experience is preferred.

Requirements

  • Previous home care or medical office experience is required.
  • 1–3 years of experience in medical records or clinical administration preferred.
  • Strong attention to detail with the ability to manage multiple tasks and priorities.
  • Knowledge of healthcare regulations, home care compliance standards, and EMR systems.
  • Excellent verbal and written communication skills.
  • Proficient in Microsoft Office and standard office equipment.
  • Ability to work independently and collaboratively in a dynamic environment.
  • Valid driver's license, reliable transportation, and ability to travel as needed.

Nice To Haves

  • Previous Home Health Aide (HHA) or Certified Nursing Assistant (CNA) experience is preferred.

Responsibilities

  • Ensure all client information is entered accurately and maintained within the EMR.
  • Maintain organized electronic and paper medical records.
  • Monitor data integrity and ensure documentation meets agency and regulatory standards.
  • Process and fax physician orders daily.
  • Track and follow up on outstanding physician signatures and missing documentation.
  • Ensure timely completion of required clinical records.
  • Perform Level 1 quality reviews of clinical documentation.
  • Verify that required forms are complete, accurate, and properly filed.
  • Assist with maintaining agency compliance and quality initiatives.
  • Monitor EMR clinical workflows and system widgets.
  • Assist Clinical Managers with special projects and operational needs.
  • Support agency initiatives focused on quality improvement and regulatory compliance.
  • Communicate professionally with physicians, referral sources, providers, and interdisciplinary care teams.
  • Obtain necessary medical histories and clinical documentation.
  • Serve as a resource for office staff and clinical teams.
  • Provide quality Home Health Aide services to clients as needed to ensure continuity of care.
  • Follow individualized care plans while delivering compassionate, safe, and professional personal care.
  • Maintain appropriate documentation for all client visits.
  • Answer incoming calls promptly and professionally using agency scripting.
  • Assist with front office operations during high-volume periods.
  • Provide exceptional customer service to patients, families, referral partners, and staff.
  • Maintain strict patient confidentiality.
  • Follow all HIPAA requirements, agency policies, and state and federal regulations.
