About The Position

Under the direction of the allcove Clinical Manager, the allcove Clinical Coordinator is responsible for coordinating clinical services and youth care, managing referrals and documentation, and providing client support to meet the needs of young people ages 12–25 at the allcove Youth Wellness Center, while ensuring youth voice in service planning, delivery, and program development, and serving as the on-site clinical lead in the manager’s absence.

Requirements

  • Master’s degree or post-secondary education in social work or marriage and family counseling.
  • Three years of experience in care coordination, referrals, and client support for youth in a clinical or community health setting.
  • Experience includes conducting client assessments, assessing for mental health concerns, formulating care plans, coordinating programs and managing client cases specifically for the youth ages 12 – 25.
  • Demonstrated understanding of the challenges and experiences of young people from diverse backgrounds including BIPOC, LGBTQ+ and unhoused young people.
  • Knowledge of clinical theory, adolescent and young adult development, systems theories, strengths-based perspectives, normal and abnormal psychology, group process, and counseling modalities.
  • Knowledge of laws governing mandated reporting, consumer protection (California Board of Behavioral Sciences policies), and the Tarasoff Law.
  • Knowledge of ethical practice, limits of scope of practice, and allcove Beach Cities guidelines, policies, and procedures.
  • Broad knowledge of local services for young people across health, education, employment, and youth sectors.
  • Communicates effectively with youth, families, and providers in individual and group settings.
  • Applies sound methods to plan and track work, appointments, and commitments.
  • Possesses appropriate expertise and skills to perform professional-level duties in service coordination, referral, documentation, and client support.
  • Has excellent working knowledge of computers, including Microsoft Office (Word, Excel, Outlook, PowerPoint), and the ability to learn electronic health record and referral-tracking databases.
  • Must be able to pass a background check, tuberculosis test, and drug screen.
  • Must have a valid Class C California driver's license and be able to maintain insurability under the City's Employee Use of Automobiles Policy.
  • Must have completed Mental Health First Aid certification or complete training upon hire.

Responsibilities

  • Serves as on-site clinical lead, manager of the day and on-site point of contact for coordinating daily client services and supports operations when the Clinical Manager is absent. Provides oversight to and supports Peer Specialists, staff, and partner providers.
  • Supports client safety, facilitates risk assessments and coordinates emergency services in accordance with protocol.
  • Conducts intake screenings and coordinates assessments and reassessments to determine what services are required to meet client’s needs. Coordinates client assignments to service streams, contracted clinicians and medical providers.
  • Provides care coordination, clinical oversight and on-site client support to an assigned caseload of individuals and families. Coordinates psychotherapy, workshops, and supportive therapy groups.
  • Assists with care plan development, recommends referrals, and coordinates service implementation. Monitors service delivery and client progress. Provides updates on status of cases during case conference.
  • Identifies and coordinates client support systems; supports clients and caregivers; provides information and referrals either within allcove or through community linkages; provides client advocacy.
  • Coordinates and supports interdisciplinary team meetings to facilitate coordinated client care. Works collaboratively with contracted clinicians and medical providers on-site.
  • Maintains confidentiality and accurately enters and maintains participant records and electronic health records in datacove, allcove’s centralized data system, in compliance with HIPAA standards.
  • Provides information and support to youth and families to advocate for their needs. Builds and maintains networks of youth-serving community resources and referral partners. Develops and maintains networks with youth-focused community organizations and advocacy groups.
  • Assists with education and training initiatives for staff and young people engaged in youth participation and supports the onboarding of all Peer Specialists.
  • Coordinates requests for information and referrals, including related data collection and analysis.
  • Exhibits clear understanding of the history, mission, and vision and serves as an ambassador of Beach Cities Health District.
  • Exemplifies BCHD’s core values of compassion, integrity, accountability and excellence in every interaction with staff, volunteers and members and the public.
  • Performs assigned training, duties, and work schedule as a disaster service worker on behalf of BCHD.
  • Special projects and other duties as assigned.
