About The Position

The Fort Washington Men’s Shelter is a 200 bed 24/7 emergency shelter for homeless men diagnosed with mental illness and/or substance use disorders. Our comprehensive services include case management, individual and group counseling, recreational activities, and housing placement assistance. Additionally, an on-site medical, psychiatric, and dental clinic operates Monday through Friday. The On-Site Rehabilitation Services consists of two approaches-Team PRIDE and Team STEPS. These programs provide assessment and case management services targeted at reducing the length of stay in the shelter and facilitate the transition to community-based housing. With a focus on housing placement, individual and group counseling is used to assist clients in enhancing their adult living skills, and connecting to community-based healthcare, mental health, and substance abuse services.

Requirements

  • Bachelor’s degree in social work , public administration, psychology, education, sociology, occupational or physical therapy, or related human services field with a minimum of three ( 3) years of experience
  • Two (2) years having been in a managerial or supervisory capacity.
  • Master’s degree in social work (LMSW) from an accredited college or university with a major in human services preferred
  • Leadership: Demonstrated ability to manage and motivate staff to accomplish stated program goals and objectives while developing their individual and group skills.
  • Team building: Ability to develop and foster, through demonstration, team spirit, enabling staff to overcome the challenges of providing services needed to a special client population.
  • Communication: Excellent oral, writing, and listening skills are necessary.
  • Organizational: The ability to work well within a highly pressurized, deadline environment, while meeting the short- and long-term mandates of the program.
  • Must collaborate with our Performance Evaluation and Quality Assurance Department (PEQA) and develop/ exhibit the ability to independently evaluate and manage program performance using data.
  • Interpersonal: An ability to interface with clients as well as all levels of staff.
  • Computer: Knowledge of case management software as well as proficiency in Microsoft Office Suite

Responsibilities

  • Under the direction of the Shelter Director, with wide latitude for independent action and decision making, the Clinical Coordinator is responsible for the overall coordination and delivery of services to clients assigned to the on-site rehabilitation program.
  • Works collaboratively with the Shelter Clinical Director to ensure the on-site rehabilitation staff provides high quality case management and housing placement services to clients.
  • Directs staff in providing assessment, counseling and referral services to the clients participating in the Team PRIDE and Team STEPS initiatives, ensuring the needs of clients are being met and program objectives are being achieved.
  • Day to day Supervision of programming and service delivery to approximately 200 clients.
  • Provide clinical and task supervision to a staff of (7) case managers (2) peer counselors (1) senior case manager.
  • Review and sign-off on documentation completed by the case management team and monitor appropriateness of the standards of care.
  • Oversees the data in the DHS CARES system, ensuring that the data is current and that case notes, ILP’s and referrals are being provided by the staff.
  • Provides guidance and direction to (2) peer counselors ensuring post placement follow –ups are conducted.
  • Conducts Case Conferences with clients and Case Managers on a regular basis.
  • Maintain program statistical data for DHMH and DHS.
  • Coordinates the Money Management Program exercising fiduciary responsibilities for the clients to ensure savings goals are being achieved and funds are being spent responsibly by the clients.
  • Provides leadership during crisis/trains staff in protocols to be followed in clinical emergencies.
  • Attends and participates in weekly shelter meetings on clients and housing placements.
  • In consultation with the Director, plan and implement QA activities.
  • Performs other duties as assigned by the Shelter Director.
  • Available on call for emergencies on a 24X7X365 basis, especially after regular work hours and on weekends.
