About The Position

Cornerstone Family Healthcare is actively recruiting for a Clinical Coordinator to join our growing team in Middletown, NY. This is a full-time position with a rate of pay between $36.00 - $41.00 per hour. The general purpose of this role is to provide oversight of the nursing and clinical care provided in the department.

Requirements

  • Successful completion of LPN program from an Accredited School
  • BLS Certification
  • NYS Education Department Licensed Practical Nurse License

Responsibilities

  • Provide daily oversight to the department’s LPNs and Medical Assistants under the supervision of the Lead Nurse.
  • Provide supervision to the department’s LPNs and Medical Assistants.
  • Create, implement, and supervise the new nurse/MA orientation process in collaboration with the Clinical Training Coordinator and Lead nurse.
  • Ensure that nursing staff adhere to CORNERSTONE nursing policies.
  • Participate in CORNERSTONE’s Care Team Model, including Pre-Visit Planning, leading care team huddles/communication, directing population management activities for assigned patient panel, and directing/participating in quality improvement activities.
  • Manage patient flow based on department volume and provider needs.
  • Obtain assistance from Lead Nurse and/or Practice Manager for staffing issues.
  • Coordinate the departmental nursing schedule to ensure adequate coverage at all times, reporting coverage issues to the Lead Nurse or Practice Manager.
  • Complete daily assignments of nurses and medical assistants to providers and schedule lunch breaks.
  • Ensure the DEPARTMENT PROXY in Centricity is assigned and addressed promptly, with no outstanding phone notes/messages or orders concerning laboratory follow-up.
  • Ensure the department is stocked with necessary equipment, supplies, and forms, submitting supply requests to the Lead Nurse.
  • Maintain inventory of all drugs and biologicals, logging their use as required.
  • Maintain inventory of sample medications and complete required logbooks.
  • Perform monthly inventory of all medication (stock and samples) to check for expiration dates and recalls.
  • Monitor the crash cart for daily and monthly inspections, ensuring it is locked and fully stocked.
  • Receive all laboratory and diagnostic procedure results and promptly address any follow-up needed.
  • Answer and respond to nurse telephone calls, including triage/clinical advice, prescription refills, requests from hospitals and other agencies, and general patient requests.
  • Provide triage of walk-in patients under the supervision of a provider or Lead Nurse.
  • Complete patient screening and provider assistance, including vital signs, chief complaints, assisting providers with in-office procedures, administering medication and treatments, vaccinations, hearing and vision screenings, and patient education.
  • Carry out Medical Providers' orders for EKGs, emergency response, medication administration, vaccinations, prescribed treatments, and other procedures.
  • Ensure all CLIA waived testing performed on-site has been quality controlled and logged.
  • Perform monthly cleaning and maintenance of all point of care testing analyzers.
  • Assist in performing Lead Nurse responsibilities in their absence, under the supervision of an RN or provider.
  • Assist the Clinical Training Coordinator and Lead Nurse with yearly nursing staff evaluations and periodic evaluations.
  • Be familiar with and adhere to all clinical, administrative, and general policies and procedures.
  • Attend and participate in monthly department, All Staff, and other required meetings, as well as staff huddles.
  • Maintain confidentiality of all aspects of CORNERSTONE, including patient confidentiality, financials, and employee relations.
  • Perform other duties as assigned.
