About The Position

Provides administrative support services to clinical operations as well as the team leaders. Provides phone support to the reception area and assists patients and families with their needs to achieve the desired outcome of comfortable and peaceful death for patients. This is a full-time, M-F, 8am-5pm located at our Wooster office. Will work in collaboration with our other two team finical coordinators. We provide superior care and superior services to patients at their end of life journey. Only those who have a heart for hospice will succeed.

Requirements

  • Associate degree in office management preferred
  • Competent in Microsoft Word and Microsoft Excel and preferred experience with Electronic Medical Records
  • Excellent communication and interpersonal relations skills
  • Ability to drive during daytime, nighttime or inclement weather
  • Valid Driver's License with Safe Driving Record
  • State Minimum Automobile Insurance Coverage

Responsibilities

  • Prepares and distributes care manager caseload report to team leaders daily so that team leaders can obtain an accurate picture of the care manager’s current caseload to trouble shoot any problems that might occur during the day to ensure quality patient care.
  • Distributes lab work to care manager’s the same day received so that care manager’s have access to lab work in a timely manner to ensure quality patient care.
  • Maintains PTO, holiday, on-call, and attendance records for teams, as assigned so that team leaders know what staff is available to provide patient care ensuring quality of patient care.
  • Handles all aspects of scheduling assigned meetings such as contacting participants, reserving the meeting room, overseeing the room set-up and ordering refreshments as appropriate, as well as, recording minutes to ensure communication.
  • Receives patient and families’ phone calls using appropriate customer service skills and notifies the appropriate team member of the call immediately to ensure timely communication.
  • Documents phone calls in the EMR in a timely manner so that all staff caring for the patient has accurate information ensuring effective communication.
  • Prepares QUALITY OF LIFE meeting paperwork and distributes to team meeting before team so that team meetings can occur in a timely manner ensuring effective and timely communication.
  • Reviews synchronization report weekly and reports lack of compliance to Team Leader for follow up to ensure timely communication of patient information as requested.
  • Prints DME reports for each QUALITY OF LIFE team meeting which Team Leaders review with care managers to report any variances to ensure financial viability of the organization.
  • Reviews payroll for accuracy and completes payroll responsibilities for assigned department to ensure financial viability of the organization.
  • Completes audits/reports for Director/Team Leaders as needed.
  • Assists in other activities and departments when requested to ensure complete and thorough patient care for all Ohio’s Hospice patients.
  • Participates in Continuous Quality Improvement activities to support the quality of Ohio’s Hospice services
