About The Position

The primary purpose of this position is to provide clinical assessments, intensive case management, and referral program management. The incumbent will facilitate collaboration between the HACP, residents, and agencies and organizations serving HACP residents. Collaborate with HACP Property Managers, HACP Projects, Resident Self Sufficiency staff, and service providers to assist in resident self-sufficiency. The incumbent is responsible for developing a preliminary treatment plan, monitoring linkage and progress, and completing required clinical documentation related to individual caseloads in a timely manner. All activities must support the Housing Authority of the City of Pittsburgh (“HACP” or “Authority”) mission, strategic goals and objectives.

Requirements

  • Master’s degree in Social Sciences, Counseling, Psychology Social Work or a related field
  • A minimum of 2 years’ experience working in clinical coordination, clinical consultant, therapist or social services.
  • Will consider other equivalent combination of education and work experience, which will result in the required skills, knowledge and abilities.
  • PA Driver’s License and use of an automobile during work hours required.
  • Must be able to work evenings and weekend.
  • Strong computer skills
  • Proficient in Microsoft Office applications (MS Word, Excel, and Outlook).
  • Ability to learn other computer software programs as required by assigned tasks.
  • Proficient in completion of psychosocial assessments
  • Must be able to make sound decisions
  • Sound customer service skills

Responsibilities

  • Performs psychosocial assessments, standardized assessments, and/or outcome based measures.
  • Monitors HACP referrals, assessments and linkage to independent service programs; maintaining data, and preparing reports on service performance.
  • Develops preliminary, individualized, treatment plans following standard regulations to ensure best practice and appropriate level of care referral is made.
  • Responds to emergency/crisis situations with consumers, assists with ensuring resident safety, and provides appropriate care recommendations.
  • Attends supervision with manager and receives clinical consultation to ensure continuum of care and best practices are maintained.
  • Coordinates and facilitates cooperation, coordination, and collaboration between and among community serving programs, organizations, and agencies, including HACP programs.
  • Provide technical support and training to tenant groups to build strong, self-sufficient councils.
  • Resolve problems, answer questions, identify referrals and otherwise perform necessary decision-making and problem-solving functions related to HACP program facilities and resident service programs.
  • Develop working relationships with partner organizations such as Department of Health and Human Services, Department of Welfare, and others.
  • Performs other duties as assigned.
