About The Position

A Clinical Coordinator supports high-quality, patient-centered acute care through clinical expertise, established practice standards, and coordination of safe care transitions. The Hospital Care at Home Clinical Coordinator screens ED and inpatient candidates for eligibility, supports timely enrollment, and coordinates seamless transition to home-based care. This role educates patients, families, and care teams to ensure understanding of the program. The Hospital Care at Home Clinical Coordinator represents the team in interdisciplinary case conferences, partnering with physicians, nursing, and care management to identify appropriate patients, promote utilization, support continuity of care, and maintain focus on quality, safety, and patient experience.

Requirements

Associate’s Degree in a Human Service-related field
May accept 5+ years of related care coordination experience in lieu of an Associate's Degree.
LPN license

Bachelor’s Degree in Science or Arts in Human Services related field

Responsibilities

Screens ED and inpatient candidates for eligibility for home-based care.
Supports timely enrollment into the home-based care program.
