About The Position

The Clinical Coordinator/Residential Coordinator (CC/RC) reports to the Program Director and is responsible for the coordination of all clinical services and the day-to-day operation and customer service of the residential facilities. This role involves supervising Direct Support Professionals (DSPs)/Behavioral Support Professionals (BSPs) in group homes, ensuring compliance with training regulations and personnel file requirements, and managing staffing hours, overtime, and expenses. The CC/RC is also responsible for maintaining the physical structure, cleanliness, and curb appeal of the group home, including vehicle maintenance. This position requires on-call availability 24/7, including shared weekend on-call status. Responsibilities include running doctor appointments, ensuring preventative health care, coordinating with Clinical Coordinators to meet regulations, conducting drills, grocery shopping, supervising staff, interacting with stakeholders, participating in Person-Centered Plan (PCP) meetings, monitoring staff adherence to plans, and serving as a role model for person-centered principles. The role encompasses coordination of PCPs, medical appointments, assessments, new admissions, behavioral support services, data collection, employee training, protocol development, budget management, scheduling, working shifts as needed, and maintaining a safe program environment in accordance with organizational policies.

Requirements

  • High School diploma with at least 5 years of experience working in the field of intellectual or developmental disabilities; or an Associate’s degree in a human services field with at least 2 years of experience working in the field of intellectual or developmental disabilities; or a Bachelor’s degree in a human services field (preferred).
  • Must maintain certification in CPR and First Aid.
  • Must be able to work a 40 hour weekly flexible schedule in an effort to meet the demands of the individuals in the home.
  • Ability to be on- call 24 hours a day, work a flexible shift, and maintain expected standards of work production.
  • Participate in weekend on-call rotation.
  • Attend training activities, meetings, and routine supervision with the supervisor.
  • Must be highly organized, computer literate, able to read and write English.
  • Ability to safely operate a motor vehicle to transport oneself, individuals, and program supplies as necessary.
  • The physical ability to travel to assigned locations, stand, stoop, bend, reach, pull, push, lift, grasp, talk, see, hear and perform basic and light home maintenance activities, and operate office equipment.
  • Move and lift light objects up to 30 pounds such as mail, supplies, files, and equipment.
  • The ability to remain in a sitting position for extended periods of time.
  • Occasionally, ability to lift, carry, push, pull or otherwise move objects, including the human body.

Responsibilities

  • Coordination of all clinical services and day-to-day operation of residential facilities.
  • Supervising DSP/BSP staff in the group home.

  • Ensuring work orders are completed properly, timely, and according to expectations.
  • Ensuring the home is odor-free and maintains excellent curb appeal.
  • Ensuring the yard is nicely kept, lawns mowed, bushes and trees pruned, leaves removed, etc.
  • Maintaining the proper maintenance and cleanliness of company vehicles assigned to the group home.
  • Running doctor appointments with individuals in the home and ensuring they attend preventative health care and dental appointments.
  • Working with Clinical Coordinators to ensure licensing and regulatory compliance.
  • Conducting fire drills, safety and evacuation drills.
  • Grocery shopping for the home.
  • Providing overall supervision for the smooth running of the group home.
  • Interacting with family members, support coordinators, and other stakeholders.
  • Actively participating in the individual's PCP planning meeting.
  • Monitoring staff to ensure they are following each person's plans as written and intended.
  • Serving as a role model for staff, emphasizing and exhibiting person-centered principles.

  • Ensuring that Direct Support Professionals (DSPs) are trained on Individual PCP plans and goals by assisting with the development of competency-based trainings and examinations.
  • Serving as a mentor/role model to staff and individuals receiving services.
  • Promoting the feeling of trust, dependability, and a stable work environment to encourage staff productivity.
  • Collecting and reviewing all necessary information, assessments and reports to support development of annual person-centered plans.
  • Ensuring all clinical documentation and other information is gathered and is in place within designated time frames.
  • Coordinating the entire clinical component of services.
  • Ensuring Person Centered Plans meet the hopes, dreams, aspirations and capabilities of the supported individual and focuses on individualized outcome achievements.
  • Auditing medication administration record (MAR) weekly for accuracy and proper documentation.
  • Completing medication error reports as necessary.
  • Ensuring specialized support elements are in place.
  • Informing and maintaining communication with service coordinators, families, on issues related to general medical concerns, illnesses, injuries and incidents.
  • Encouraging skill building in meal preparation and portion sizes for all people supported.
  • Monitoring staff for compliance with menus.
  • Working to develop healthy, person-centered menus and providing daily options for exercise and movement.
  • Developing monthly activity calendars that are person centered.
  • Continuously working to connect people in their community to build natural supports.
  • Formally and informally recognizing holidays, special occasions and achievements of individuals with meaningful, appropriate celebrations.
  • Establishing effective communication efforts to individuals, families, staff through conversations and routine meetings.
  • Ensuring staff speak in a positive manner, using positive language which respects the supported individuals.
  • Ensuring benefit/entitlement programs are maximized and remain intact (Medicaid, Medicare, etc.).
  • Ensuring all billing census sheets are accurate and submitted in a timely fashion.
  • Supporting individuals in learning how to budget their finances.
  • Conducting quarterly record audits via random samplings.
  • Ensuring information within the records is accurate and up-to-date.
  • Ensuring all staff observe confidentiality and HIPAA standards.
  • Supporting the development of individualized behavioral support services.
  • Ensuring data collection is reviewed and interpreted monthly and submitted to the Behavior Specialist.
  • Understanding that behavior is communication and working to improve individual communication efforts.
  • Assisting in training staff on proper behavioral interventions and documentation where necessary.
  • Participating as needed in providing documents for Human Rights Committees, serving as the primary contact to the LHRC/HRC/BMC/Incident Management Coordinator/Incident Review Committee and providing all relevant clinical information.
  • Ensuring rights are protected and maximized, and rights restrictions are minimized.
  • Ensuring accurate and timely incident reporting and cooperation with all investigations both internal and external.
  • Working with staff to understand their roles as incident report writers.
  • Reviewing and completing all incident reports, and following internal and external incident reporting policies.
  • Assisting staff in understanding their role as mandated reporters.
  • Consistently monitoring environments for abuse and neglect indicators.

  • Completing payroll of assigned staff and completing payroll adjustments.
  • Maintaining a safe, secure and healthy work environment for all persons served, staff, volunteers, and visitors.
  • Reporting and documenting unsafe conditions to the Clinical Coordinator.
  • Providing direct support to persons served, as needed, to meet ratio needs.
  • Delegating and ensuring the replenishment of program supplies.
  • Ensuring the ongoing maintenance of program van.
  • Submitting work orders, as needed.
  • Facilitating monthly staff meetings in conjunction with QIDP.
  • Providing support as necessary to ensure needs as identified by the Inter-Disciplinary Team are met including all quality of life issues.
  • Communicating and coordinating with families, members of the Inter-Disciplinary Team (IDT), the consumers’ day program and other providers of services and supports as necessary and appropriate.
  • Supervising, scheduling, training and evaluating the Direct Support Professionals assisting in the home.
  • Hiring staff in accordance with company guidelines.
  • Performing crisis intervention; providing emergency counseling assistance.
  • Planning, coordinating, and participating with individuals in social activities.
  • Participating in IDT planning, including assessment.
  • Inspecting the home and reporting and resolving repairs and maintenance as needed.
  • Ensuring fire and other safety procedures are carried out as directed.
  • Managing and acting as petty cash custodian for all client and house funds.
  • Establishing and maintaining consumer and facility files in accordance with state, local and company requirements.
  • Monitoring activities or visitors in the home.
  • Transporting individuals and/or providing mobility training to assist individuals to go to appointments, community rehabilitation program, grocery shopping, etc.
  • Ensuring program compliance with Volunteers of America Chesapeake’s policies and procedures.
  • Maintaining and improving public image for the program in the community.
  • Participating in an on-call rotation to manage after hours crises and urgent situations.
  • Carrying a cell phone 24/7 to respond to emergencies.
  • Performing other duties as assigned.
