About The Position

We have an exciting opportunity to join our team as a Clinical Coordinator PreSurgical Optimization Home (Critical Care Exp Red) - LOH. In this role, the successful candidate works with high risk patient populations to identify the need for and to coordinate services that address modifiable risk factors prior to surgery. Works with patients and caregivers to ensure that they are knowledgeable about and connected to health care services that address their individual needs. Provides patients with targeted health education and promotes preventative healthcare services. This position supports program outcomes of reduced admissions/readmissions, reduced length of stay, fewer post-op complications, and increased self-efficacy. This position works in collaboration with clinicians, pre-admission testing, and other NYULH staff to ensure the achievement of high quality outcomes for patients/caregivers.

Requirements

  • Bachelors degree in nursing
  • NY State license as a Registered Professional Nurse required
  • 3-5 years of hospital nursing experience required, in an acute care medical-surgical unit, critical care or post-anesthesia care unit required.

Responsibilities

  • Develops and coordinates individualized care plans that incorporate appropriate interventions using established protocols based on the patient's health and psychosocial needs determined through screening
  • Educates patients on the importance of preventive care and facilitates the receipt of appropriate preventive care services
  • Responsible for the implementation and oversight of all clinical aspects impacting patient care transitions, community-based health and wellness, and management of health care needs/development of services for selected patient populations for Total Joint patients
  • Provide strategies for engaging clinicians to address patient and population health needs
  • Collaborate with clinicians from acute inpatient, rehabilitation services, and community-based practices to coordinate care and integrate components of care management for effective populations health management
  • Ensures connectivity in hang-off communication between and across clinicians and patients/families
  • For health plan/other patients receiving outreach for needed preventive services, facilitate closure of gaps in care and encourage use of in-network services
  • Facilitates referrals to other clinicians within the NYU Langone Health system
  • Coordinate and assign all necessary optimization-related pre-operative referrals according to defined criteria, including location, transition models indicated, and special requirements as needed
  • Works in collaboration with Orthopedic attendings and surgical team to review comprehensive risk assessments on all patients who present to the Orthopedic clinic for a TKA or THA
  • Formulates, plans, and implements populations based high risk patient screening process to ensure effective and timely patient health care transitions and services in the perioperative period
  • Coordinate and enroll identified high risk patients into one or more risk modification programs
  • Prepares patients for surgical appointments including health promotion interventions, such as establishing proper BMI and weight counselling, smoking cessation, and tobacco use counseling, evaluates suitability of current housing, and other social determinants of health that may impact surgical outcomes.
  • Works with other members of the patients care team as appropriate to ensure activities are integrated and non-duplicative
  • Considers the needs and behaviors of specific patients/caregivers in a culturally competent manner; incorporate clinical knowledge and related experience in communications
  • Facilitate collection of patient outcomes data. Review and monitor outcomes, program objectives, and evaluate/revise practices as needed
  • Seeks guidance as appropriate and takes direction from Manager.
  • Comply with policies and procedures associated with each department initiative.
  • Documents activities clearly and comprehensively in EMR in accordance with departmental standards
  • Attend and participate in program/initiative teleconferences, program enhancement trainings and meetings, as required
  • Meet all other expectations and responsibilities of the program/initiative.
