Wentworth Health Partners (WHP), an affiliate of Wentworth-Douglass Hospital and member of Mass General Brigham Medical Group, offers Endocrinology and Diabetes Care at our state-of-the-art facility at 10 Members Way in Dover, NH. This practice provides care for patients with endocrine conditions, including diabetes, thyroid disorders, and a broad spectrum of other hormonal disorders. Our expert team of clinical and administrative professionals focus on specialized diagnostics and treatment, including advanced patient education programs to assist individuals looking to improve their health and enjoy a better quality of life. We are seeking a 40-hour Clinical Coordinator LPN to join our Endocrinology team! The practice is open Monday through Friday from 8:00am to 6:00pm, and schedules are staggered accordingly based on practice needs. Responsible for managing the daily operations of a unit or department including scheduling, managing staff shifts, assigning nursing staff to patients, monitoring charts, coordinating treatment for patients with different physicians and medical providers, and ensuring each department runs smoothly. Requirements

  • Associate's Degree Nursing required
  • Nursing Experience 2-3 years required
  • Hospital Setting Experience 0-1 year required
  • Knowledgeable in general nursing practices and associated departmental or unit responsibilities.
  • Ability to assess patients, interpret tests and recommend the most efficient treatment plan and services.
  • Excellent planning and problem-solving skills.
  • Ability to communicate effectively with patients, families and hospital staff.
  • Excellent time management skills.
  • Ability to communicate and resolve issues effectively with a broad population of patients, staff and physicians.

Responsibilities

  • Create patient reports and monitor the status of treatment.
  • Draft discharge papers for patients.
  • Review patient charts.
  • Oversee staff scheduling and assignments.
  • Coordinate treatment for patients from multiple health care providers.
  • Communicate medical updates and treatment plans with patients and family.
