The Clinical Coordinator - Care Manager Medicaid is responsible for providing comprehensive care management services to multiple populations, including those with disabilities, pregnant and postpartum members, individuals with chronic conditions, children eligible for special healthcare services, children in foster care, former foster care youth, members undergoing Cell and Gene Therapy, and those receiving mental or behavioral health services. This role involves serving as a single point of contact for members, conducting face-to-face visits in various settings, assessing health status and needs, developing personalized care plans, and coordinating the delivery of services. The position requires identifying barriers to care, advocating for members, collaborating with support teams and healthcare providers, and ensuring compliance with organizational policies, NCQA standards, and regulatory requirements.
