Tallahassee Memorial Healthcare and FSU Health are looking a Clinical Community Wellness Coordinator in the Panama City Beach area. This role is a Part-time role with potential to become Full-time. Develops effective communication and education of Tallahassee Memorial and FSU Health services to physicians, facilities, and individuals who may need information about Tallahassee Memorial HealthCare (TMH) services in Panama City Beach. Leads program and initiative design efforts including identification and development of goals, objectives, operational/research metrics and coordination of participating clinical and non-clinical resources. Behavioral Criteria: Establishes initiative-specific priorities, and directs coordination of interdepartmental resources/services. Collaborates with key individuals at TMH and develops metrics relative to Wellness initiatives. Reports on campaign activities and milestones. Evaluates aggregate health risk assessment and biometric data to determine priorities to be addressed and the type and number of educational programs needed. Assembles data to design an annual and multi-year wellness strategic plan Provides leadership throughout the organization for the implementation of health and wellness strategies and initiatives Develops and publishes internal communications regarding health and wellness, while working with TMH communications (PR) for branding, review, approval and distribution of broad-based materials in a multi-channeled approach. Designs and implements promotional and motivational wellness programs, manages incentives and coordinates wellness activities that may include seminars, classes, incentive programs, contests, newsletters and committees. Develops and supports projects for the environment of wellness at all TMH campuses for healthy foods, physical activity, stress management and balancing work/family Build community momentum, motivation, and participation in health and wellness activities. Coordinates and help to adapt existing health education and prevention related programs and services. Support the planning, implementation, and evaluation of TMH for Life programs and activities. Develop meaningful community partnerships and represents TMH in the wellness community Support the implementation and management of transactional care programs and other programs focused on high risk populations Work with other community partners to distribute health promotion resources and facilitate health screenings and wellness activities
