About The Position

Gateway Rehab Center has an immediate need for a Clinical Case Manager located at our Center Township, PA location. You will work directly with our residential patients. The Case Manager will be responsible for completing assessments and treatment plans, facilitate group sessions, and help facilitate movement to the next level of care.

Requirements

• High School or GED.
• Computer literacy is required as part of the electronic medical record.
• Excellent people skills and organizational skills.
• Valid driver’s license with favorable driving record.
• Obtain CPR and First Aid certification.
• Pass PA Criminal Background Check.
• Obtain Childline and FBI clearances.
• Pass Drug Screen
• 2-Step TB Test

Nice To Haves

• Bachelor’s Degree preferred.
• Understanding of 12 step recovery programs and the recovery process preferred.

Responsibilities

• Provides case management support to the inpatient clinical team.
• Completes all documentation in patients’ charts regarding case management and treatment-related activities.
• Facilitates group sessions when needed.
• Networks with referral sources and outside treatment providers and other agencies.
• Coordinates and communicates with other Gateway programs and departments.
• Provides oversight of patient daily activities as needed, including unit supervision.
• Provides direct patient care as needed.
