Requirements

  • Bachelor’s degree or higher in social work or related field, is required.
  • One to two years of related experience is required.
  • Must be at least 21 years of age.?
  • Willingness to submit to record checks to detect criminal convictions of behavior harmful towards children, spousal abuse, distributing drugs or sex crimes.
  • Requires a valid driver’s license/auto-insurance or the ability to obtain one with approved driver status.

Responsibilities

  • Assess youth and family needs and connect clients with appropriate services, supports, resources, and planning opportunities in alignment with LSSSD programs and services.
  • Provide case management and supportive services to youth placed at the Arise Youth Center as a community-based alternative to detention.
  • Collaborate extensively with youth, families, referral sources, law enforcement, schools, social service agencies, and other community partners to coordinate services and support positive outcomes.
  • Facilitate access to legal, financial, medical, mental health, educational, and other community resources as needed.
  • Provide ongoing support, crisis intervention, follow-up services, and referrals to address identified needs.
  • Maintain accurate and timely documentation, reports, and case records in compliance with agency, state, and federal requirements.
  • Promote program awareness and maintain referral relationships throughout the community.
  • Develop and maintain positive working relationships with staff, youth, families, and community stakeholders, and actively participate in treatment team meetings, case reviews, staff meetings, and other agency activities.
  • Monitor client progress, advocate for client needs, and support the development and implementation of individualized service plans to promote safety, stability, and successful outcomes.
