Requirements

Bachelor’s degree or higher in social work or related field, is required.
One to two years of related experience is required.
Must be at least 21 years of age.?
Willingness to submit to record checks to detect criminal convictions of behavior harmful towards children, spousal abuse, distributing drugs or sex crimes.
Requires a valid driver’s license/auto-insurance or the ability to obtain one with approved driver status.

Responsibilities

Assess youth and family needs and connect clients with appropriate services, supports, resources, and planning opportunities in alignment with LSSSD programs and services.
Provide case management and supportive services to youth placed at the Arise Youth Center as a community-based alternative to detention.
Collaborate extensively with youth, families, referral sources, law enforcement, schools, social service agencies, and other community partners to coordinate services and support positive outcomes.
Facilitate access to legal, financial, medical, mental health, educational, and other community resources as needed.
Provide ongoing support, crisis intervention, follow-up services, and referrals to address identified needs.
Maintain accurate and timely documentation, reports, and case records in compliance with agency, state, and federal requirements.
Promote program awareness and maintain referral relationships throughout the community.
Develop and maintain positive working relationships with staff, youth, families, and community stakeholders, and actively participate in treatment team meetings, case reviews, staff meetings, and other agency activities.
Monitor client progress, advocate for client needs, and support the development and implementation of individualized service plans to promote safety, stability, and successful outcomes.
