Under the direction and supervision of the Clinical Supervisor for Crisis Services, the Clinical Case Manager works with children ages 5 – 17 in Crisis Services programs at St Joseph’s Villa. Crisis Services includes both the Crisis Receiving Center (CRC) and the Crisis Stabilization Unit (CSU). The Clinical Case Manager coordinates the overall services provided to individuals with external agencies and provides targeted discharge linkages for individuals to ensure ongoing stabilization for the next appropriate level of care. All aspects of service delivery are to be conducted in alignment with the Crisis Now Model – No Wrong Door/No Force First approach and systems-based framework with strong collaboration skills with and in support of other services offered by St. Joseph's Villa and the community (partnership with Region 4). All work is performed in accordance with the highest professional ethical standards, and in alignment with Saint Joseph’s Villa mission, values, and guiding principles. The Crisis Now Model ensures all people requesting services regardless of age, etc. are to briefly be screened and linked through a warm hand-off to appropriate level of care.
