About The Position

Activate Care is seeking a Clinical Support Supervisor to provide clinical guidance and escalation support to Community Health Workers delivering the Utah LIFT Path Assist for Chronic Disease program. The role helps ensure CHWs can support patients with chronic conditions while staying within the appropriate non-clinical CHW scope of practice. The position provides case consultation, reinforces evidence-based chronic disease self-management, supports clinical escalation protocols, strengthens coordination between CHWs and participating Federally Qualified Health Center (FQHC) clinical teams, and contributes to training and quality improvement. It also supports workflows for chronic disease education, telehealth access and preparation, appointment follow-through, and appropriate post-visit follow-up. The role is hybrid, combining remote work with regional travel to partner FQHC clinic sites in Utah as needed. A valid driver's license and reliable personal transportation are required. The position requires an active, unrestricted Utah license in a listed clinical discipline or related certification and experience supporting adults with chronic conditions and working in care management, population health, primary care, chronic disease management, or similar programs.

Requirements

  • Current, active, unrestricted Utah license as a Registered Nurse (RN), Licensed Practical Nurse (LPN), Licensed Clinical Social Worker (LCSW), Board Certified Behavior Analyst (BCBA), or related certification.
  • Clinical experience supporting adults with chronic conditions such as diabetes, hypertension, cardiovascular disease, obesity, COPD, or asthma.
  • Experience in care management, population health, primary care, chronic disease management, or a similar clinical program.
  • Strong understanding of clinical escalation, patient education, health literacy, and interdisciplinary care coordination.
  • Ability to translate complex clinical concepts into practical guidance for non-clinical staff.
  • Strong coaching, communication, case-consultation, and multidisciplinary collaboration skills.
  • Ability to travel to Utah clinic locations as needed.
  • Valid driver's license and reliable personal transportation.

Nice To Haves

  • Experience in an FQHC, Medicaid, or rural healthcare environment.
  • Experience supervising, training, or providing clinical support to CHWs, care coordinators, health coaches, or patient navigators.
  • Familiarity with telehealth-enabled chronic disease management, motivational interviewing, and population-health or care-management technology.

Responsibilities

  • Provide structured case consultation and clinical guidance to CHWs when participant needs, reported symptoms, or questions exceed routine non-clinical care-coordination support.
  • Help CHWs distinguish appropriate coaching and navigation from issues requiring clinical escalation, reinforcing provider recommendations without independently diagnosing, treating, or providing medical advice.
  • Provide subject-matter expertise on diabetes, hypertension, obesity, COPD, asthma, cardiovascular risk, and other common LIFT priority conditions.
  • Support standardized CHW workflows for chronic disease education, telehealth access, appointment follow-through, and clinical escalation.
  • Coach CHWs in motivational interviewing, goal-setting, and use of approved WebMD Ignite educational content to support patient self-management.
  • Develop, maintain, and reinforce clinical escalation protocols with participating FQHCs, including appropriate clinic contacts and response processes.
  • Guide CHWs on escalating symptoms, medication concerns, abnormal patient-reported information, and safety concerns.
  • Support review of safety events, escalation failures, and near misses; recommend corrective actions and maintain boundaries between CHW services and licensed clinical care.
  • Build working relationships with FQHC clinical leads, providers, nurses, and care-management staff to align Path Assist activities with clinic workflows and provider-directed plans of care.
  • Facilitate communication with FQHC clinical teams when patient needs require escalation, including participation in case conferences, workflow discussions, and quality-improvement meetings.
  • Deliver or support CHW training on chronic disease fundamentals, clinical red flags, escalation pathways, medication-support boundaries, health literacy, and telehealth preparation.
  • Develop case-based training scenarios and review chronic disease educational materials and workflows for clinical appropriateness.
  • Support patient preparation for provider-delivered telehealth encounters and reinforce appropriate post-visit follow-up instructions.
  • Review trends in clinical escalation, appointment follow-through, telehealth completion, and chronic disease barriers to inform quality-improvement activities.
  • Collaborate with Data and Evaluation teams to review program performance, safety indicators, and clinical quality measures.
  • Comply with HIPAA, privacy, security, documentation, and organizational policies.
  • Perform other duties as assigned.
