About The Position
Activate Care is seeking a Clinical Support Supervisor to provide clinical guidance and escalation support to Community Health Workers (CHWs) delivering the Utah LIFT Path Assist for Chronic Disease program. The role supports CHWs working with patients living with diabetes, hypertension, obesity, COPD, asthma, cardiovascular risk, and other chronic conditions, while ensuring CHW work stays within the appropriate non-clinical scope of practice. The position provides case consultation, reinforces evidence-based chronic disease self-management approaches, supports clinical escalation protocols, strengthens coordination between CHWs and participating Federally Qualified Health Center (FQHC) clinical teams, and contributes to training and quality improvement. It is hybrid, combining remote work with regional travel to partner FQHC clinic sites as needed. A valid driver’s license and reliable personal transportation are required. Activate Care’s mission is to improve health equity and health outcomes. Its Community Care Record platform supports coordination between healthcare and community organizations for populations facing health-related social needs, and its Path Assist program is a technology-enabled community health worker program using a structured, evidence-based intervention.
Requirements
- Current, active, unrestricted Utah license in one of the following: Registered Nurse (RN), Licensed Practical Nurse (LPN), Licensed Clinical Social Worker (LCSW), Board Certified Behavior Analyst (BCBA), or a related certification.
- Demonstrated clinical experience supporting adults with chronic conditions such as diabetes, hypertension, cardiovascular disease, obesity, COPD, or asthma.
- Experience in care management, population health, primary care, chronic disease management, or similar clinical programs.
- Strong understanding of clinical escalation, patient education, health literacy, and interdisciplinary care coordination.
- Ability to translate complex clinical concepts into practical guidance for non-clinical staff.
- Strong coaching, communication, case-consultation, and multidisciplinary collaboration skills.
- Ability to travel to Utah clinic locations as needed.
- Valid driver’s license and reliable personal transportation required.
Nice To Haves
- Experience in an FQHC, Medicaid, or rural healthcare environment.
- Experience supervising, training, or providing clinical support to CHWs, care coordinators, health coaches, or patient navigators.
- Familiarity with telehealth-enabled chronic disease management, motivational interviewing, and population-health or care-management technology.
Responsibilities
- Serve as a clinical resource and provide structured case consultation to CHWs when participant needs, reported symptoms, or questions exceed routine non-clinical care-coordination support.
- Help CHWs distinguish coaching and navigation activities from issues requiring clinical escalation, reinforcing provider recommendations without independently diagnosing, treating, or giving medical advice.
- Provide expertise on common LIFT priority conditions, including diabetes, hypertension, obesity, COPD, asthma, and cardiovascular risk.
- Support standardized CHW workflows for chronic disease education, telehealth access, appointment follow-through, and clinical escalation.
- Coach CHWs in motivational interviewing, goal-setting techniques, and use of approved WebMD Ignite educational content to support patient self-management.
- Develop, maintain, and reinforce clinical escalation protocols with participating FQHCs, including appropriate clinic contacts and response processes.
- Guide CHWs on when and how to escalate symptoms, medication concerns, abnormal patient-reported information, and safety concerns.
- Support reviews of safety events, escalation failures, and near misses; recommend corrective actions and maintain clear boundaries between CHW services and licensed clinical care.
- Build working relationships with FQHC clinical leads, providers, nurses, and care-management staff to align Path Assist activities with clinic workflows and provider-directed plans of care.
- Facilitate communication with FQHC clinical teams when patient needs require clinical escalation, including participation in case conferences, workflow discussions, and quality-improvement meetings.
- Deliver or support CHW training on chronic disease fundamentals, clinical red flags, escalation pathways, medication-support boundaries, health literacy, and telehealth preparation.
- Develop practical case-based training scenarios and review chronic disease educational materials and workflows for clinical appropriateness.
- Support workflows that prepare patients for provider-delivered telehealth encounters and reinforce appropriate post-visit follow-up instructions.
- Review program trends related to clinical escalation, appointment follow-through, telehealth completion, and chronic disease barriers to drive quality-improvement activities.
- Collaborate with Data and Evaluation teams to review program performance, safety indicators, and clinical quality measures.
- Comply with HIPAA, privacy, security, documentation, and organizational policies.
- Perform other duties as assigned.
