About The Position

CMHA-CEI is seeking a Clinical Case Manager to provide Case Management supports to a caseload of children with intellectual and developmental disabilities. The role focuses on facilitating connection and belonging in the community. The ideal candidate will be passionate about helping individuals author their lives through person-centered planning, involving family, friends, community resources, and Medicaid supports. This position involves active consultation with individuals and their teams, collateral activities with relevant agencies, coordinating transitions, assisting individuals in maintaining benefits and entitlements, and detailed documentation. The role is responsible for carrying out all program activities in a manner that aligns with CMHA-CEI’s mission, policies, and procedures.

Requirements

  • Master’s degree in Social Work, Counseling, Psychology or closely related field required or must be obtained within 60 days of application.
  • Possession of an appropriate State of Michigan credential is required or must have applied for and be able to obtain within 60 days or hire.
  • Individuals who do not have one year of experience (professional or paraprofessional) in treating or working with persons who have a developmental disability must have a QMRP sign off on documents requiring the year of experience.
  • Ability to communicate accurately and effectively both verbally and in writing required.
  • Possession of a valid Michigan driver’s license and access to reliable transportation for job related use required.

Responsibilities

  • Provide Case Management supports to a caseload of children with intellectual and developmental disabilities with a focus on facilitating connection and belonging in the community.
  • Facilitate person-centered planning with the support of family and friends, and the utilization of community and Medicaid supports.
  • Provide active consultation to those receiving support, including children or adults living with their families, adults in supported living situations, group homes, or living independently in the community.
  • Treat to need and support people through a variety of situations and different environments.
  • Provide ongoing consultation with people and their teams.
  • Engage in collateral activities with agencies related to supports.
  • Coordinate transitions.
  • Help individuals maintain benefits and entitlements.
  • Perform detailed documentation.
  • Carry out all activities of the program/sub-unit in such a manner that fulfills CMHA-CEI’s mission, policies and procedures.
