NFI North is seeking a dedicated, experienced Clinical Case Manager to join our residential Transitional Housing Program in Bradford, New Hampshire. This program provides comprehensive support and services to adults living with severe and persistent mental health challenges, helping individuals achieve stability, and greater independence to be able to transition back into the community. The Clinical Case Manager will provide a wide range of case management services, including treatment planning, group facilitation, care coordination, and skill development. Key responsibilities include assisting residents with budgeting, accessing community resources, navigating Medicaid and other benefits, and developing the skills necessary to achieve greater independence and meet their individualized goals. This position offers a dynamic and rewarding work environment where no two days are the same. Requirements

  • Bachelor’s degree in psychology, Social Work or related field.
