About The Position

The Clinical Case Manager provides case management and clinical services to AILP clients. The primary emphasis of the position is to provide case management, therapy, crisis, treatment planning, and psychosocial assessment services to adults with mental health, substance use disorder, and transitional housing related issues in the PRC Baker Places Assisted Independent Living Program. The Clinical Case Managers partner closely with the Case Management Program Manager, Case Management Program Director, Housing and Entitlements Manager, and Quality Management to achieve an excellent standard of client care.

Requirements

  • Actively Licensed or registered with the California Board of Behavioral Sciences (BBS), as one of the following; LCSW, LMFT, Psy.D., ASW, AMFT, LPCC, APCC
  • Two (2) years clinical experience working with adults with a history of mental health (required), substance use disorder (required), and/or homelessness (preferred)
  • Strong practical and theoretical foundation in clinical MH/SUD evidence-based best practices.
  • Must possess exceptional verbal and written communication skills.
  • Proficiency in electronic medical record systems and Microsoft Office products.
  • Willingness to work non-traditional hours as the job requires.
  • Must maintain First Aid and CPR Certifications.
  • Effective time management and ability to meet deadlines.
  • Must be available to work flexible hours outside of assigned shifts occasionally.
  • Exhibit exemplary customer service, compassion, and care in the execution of all duties and interactions.

Responsibilities

  • Provide individual behavioral health counseling and case management designed to support transitional and non-transitional clients with mental health and/or substance use disorder needs.
  • Provide group therapy and rehabilitation designed to support transitional and non-transitional clients with mental health and/or substance use disorder needs.
  • Coordinates with other providers to support residents in accessing primary medical care, mental health care, substance use disorder care, and other necessary/needed services to address client’s needs– including PRC Baker Places’ wrap-around services.
  • Provide outreach to AILP clients who may otherwise not access services by traveling to the client’s residence and providing on-site services.
  • Conducts initial and updated psychosocial assessments.
  • Develops individualized treatment plans and adjusts as needed.
  • Documentation to meet agency standards and timeliness.
  • Provide culturally competent services using harm reduction and trauma informed principles.
  • Responds to mental health crisis to provide immediate access to services, de-escalation, and follow-up care.
  • Provides rotating on-call duties that may include responding to a crisis or de-escalation outside of business hours.
  • Maintain a productivity target based on contractual requirements.
  • Maintain a minimum case load of 25 clients or higher based on occupancy, client needs, and department needs.
  • If licensed, complete and maintain 51/50 certification; responding when necessary for assessment and follow-up.
  • Provide step-down case management for clients transitioning to certain housing.
  • Promote agency mission, purpose, and values in relationships with internal and external providers.
  • Maintain licensure or license eligible status, complete required annual trainings, maintain ANSA and ASAM certification.
  • Maintain CPR and First Aid certifications.
  • Comply with annual TB testing requirements.
  • Additional duties as assigned.
