About The Position

Renaissance Social Services, Inc. is recruiting an experienced Case Manager to join our Substance Use Prevention and Recovery/Outreach (SUPR) Team! Renaissance Social Services, Inc. (Renaissance) is a leader in Chicago’s response to homelessness. Renaissance prevents and ends homelessness for vulnerable Chicagoans, providing a range of proven solutions including prevention, outreach, rapid re-housing, permanent supportive housing, behavioral health care, and clinical case management. Position Summary The SUPR/Outreach Case Manager provides supportive case management and recovery-focused services to individuals living independently in scattered-site apartments across the city of Chicago. This role is part of our Substance Use Prevention and Recovery (SUPR) outreach team and delivers services in the community, meeting participants where they are. Our participants are adults living with mental health conditions such as schizophrenia, bipolar disorder, or depression. Many are also working toward recovery from drug and alcohol use, and many are managing medical conditions. The SUPR/Outreach Case Manager supports each participant's recovery goals within a harm reduction framework, at a pace set by the individual. The goal of the program is to help participants maintain permanent housing, build skills and income, and achieve greater self-determination.

Requirements

  • Must meet the State of Illinois criteria for a Mental Health Professional (MHP) via one of the following pathways: Bachelor’s degree in a human services field (Social Work, Psychology, Counseling, etc.).
  • Bachelor’s degree in an unrelated field plus two (2) years of documented clinical experience in a mental health setting.
  • High School Diploma or GED plus five (5) years of documented, supervised clinical experience in mental health or human services.
  • A minimum of 3–5 years of clinical experience, preferably in case management or direct community-based service.
  • Proven experience working with individuals experiencing chronic mental illness and substance use disorders, with a firm commitment to Harm Reduction principles.
  • Ability to relate appropriately and effectively with individuals and families, as well as with staff and personnel from other community agencies.
  • Strong computer literacy, including the ability to utilize Microsoft Office Suite and Electronic Health Record (EHR) systems for clinical documentation.

Nice To Haves

  • Active Certified Case Manager (CCM), CRSS (Certified Recovery Support Specialist), or CPRP (Certified Psychiatric Rehabilitation Professional) designation.
  • Experience in triage, crisis intervention, and rapid assessment within a housing or clinical setting.
  • Skill in organizing and documenting clinical progress notes and comprehensive service plans in accordance with HIPAA.
  • Ability to work effectively as a member of an interdisciplinary team to ensure continuity of care.

Responsibilities

  • Provide intensive, community-based case management to participants in scattered-site housing through home visits and field outreach.
  • Support participants' substance use recovery goals within a harm reduction framework, at a pace set by the individual.
  • Assess participant needs and barriers to services, and work with participants to develop and carry out individualized service plans.
  • Connect participants to community resources through information, referral, linkage, and advocacy, and coordinate services with other providers.
  • Provide crisis intervention and de-escalation when participants are in crisis, in consultation with a Qualified Mental Health Professional (QMHP), and coordinate follow-up care.
  • Work with participants to build skills, increase income, strengthen self-determination, and maintain housing stability.
  • Work cooperatively with landlords and property managers to resolve tenancy issues and prevent housing loss.
  • Plan and lead community-building, social, and skill-building activities for participants.
  • Maintain accurate, timely records in the web-based database and paper files, including the documentation and statistics required by funders.
  • Protect the confidentiality of participant information in written, oral, and electronic form, in accordance with HIPAA, 42 CFR Part 2, and Illinois law, and release information only with a valid authorization or as required by law.
  • Report all suspected abuse and neglect as required by state law.
  • Travel throughout Chicago using a personal vehicle to deliver services in participants' homes and community settings.
  • Perform other duties as assigned.
