About Center Star Assertive Community Treatment (ACT) Center Star Assertive Community Treatment (ACT) is a community-based program serving adults with serious mental illness who need intensive support to live successfully in the community. Our multidisciplinary team provides mental health treatment, medication support, substance use services, peer support, employment assistance, and housing support to help clients achieve greater stability, independence, and recovery.

Requirements

  • For Clinical CM, master's degree with two years of experience in social services; or bachelor's degree with four years of experience in social services.
  • Valid CA driver’s license and current auto insurance per contract requirements required.
  • Cultural competency understanding required.

Nice To Haves

  • For SUD CM a certification from recognized board is preferred and registration required.
  • Experience providing mental health and/or substance use disorder services preferred.
  • Bilingual preferred.
  • Knowledge of Microsoft Office and EHR software preferred.

Responsibilities

  • Provide case management with crisis intervention, discharge planning, group & individual work, mental health/substance use disorder counseling, screening, and assist clients in meeting basic needs and symptom management.
  • Complete accurate and timely documentation, meet minimum billable requirements for direct client services.
  • Facilitate placement including pre-placement visits, and transportation as needed.
  • Shift coverage including part of the on-call team.
  • Provide field-based services as required by program and client need.
  • Maintain confidentiality of client’s care.
  • Participate in scheduled program meetings.
  • Other relevant duties as assigned.
