About The Position

The Clinical Care Partner is responsible for coordinating patient care appointments, following established standards and practices of population health management. This role involves calling patients for follow-up appointments after hospital admissions and contacting high-risk patients identified by the Quality Department to address quality measures. The position relies on guidelines and experience to complete tasks and operates under the close supervision of the Clinic Manager and Quality Manager.

Requirements

• 3 years experience as a medical assistant
• Graduate of an accredited medical assistant program, or 2 years additional medical assistant experience in lieu of education.
• BLS Certification

Responsibilities

• Reviews evidenced based care measures for population of patients.
• Assists providers with quality measures to ensure care needs are outlined and established prior to visit, optimizing patients time with the Provider.
• Prepares for clinic interdisciplinary team meetings/huddles by confirming previous orders have been completed and resulted, reviewing inpatient admissions, reviewing anticipated annual screenings, preparing the agenda for meetings, printing patient care summaries for providers as needed, identifying high-risk patients, and communicating with the team for follow-up.
• Calls and schedules patients listed on the Quality Gap report and meets specified deadlines.
• Requests results from outside facilities and scans results into the patient EMR by established deadlines.
• Observes and adheres to quality standards established by the department.
• Participates in continuous quality improvement programs to foster the provision of quality health care services by the department and clinic, ensuring all services are provided with compassion, understanding, and respect for others.
• Conducts post-visit reviews on clinic wellness patients as well as specific measures with Population (Adult and Pediatric) such as uncontrolled blood pressure, etc.
• Performs other duties as assigned or requested.
