The Clinical Care Coordinator is responsible for providing care coordination and community outreach under the Alabama grant. This position focuses on both community involvement and managing a caseload of clients, documentation and charting in electronic health record, communicating with multidisciplinary teams, educating community on available programming, and ensuring adequate patient care, with a main focus on veterans, military and their families receiving quality outpatient counseling services. Military cultural competency and a well-developed understanding of community resources for outpatient counseling is crucial. The care coordinator should be engaging and energetic while maintaining professional relationships with the community they will serve and are capable of engaging the Veteran and civilian community using a variety of modalities including online, virtual, and in- person means. Candidates must live in the state of Alabama, with a preference for Huntsville and Fort Rucker. This position will require attendance at outreach, community events and in-person meetings.
