About The Position

Under indirect supervision, coordinates and provides comprehensive patient care and associated services within a defined specialty practice. Assesses patient/family needs; coordinates care and associated procedures or services for complex patients; provides education to patient/family; provides telephone advice to patient/family, based upon protocol; functions as an advocate and resource for patient/family & health care team; and facilitates communication within health care team. Mentors staff & delegates tasks to staff within the practice, collaborates with other health care providers, schools and/or community services to meet patient/family needs.

Requirements

  • Graduate of an accredited Registered Nurse Program.
  • Current State of Connecticut Registered Nurse licensure required and maintained thereafter
  • Current Basic Life Support (BLS) / CPR in adult, child & infant required and maintained
  • Self-directed, autonomous, able to perform tasks with minimal oversight
  • Working with clinical providers as a part of the multidisciplinary team
  • Excellent written and verbal skills, time management and organizational skills
  • Computer technology- experience with electronic medical records, familiarity with excel and data entry into program databases
  • Patient/family teaching skills
  • Competence in providing developmentally appropriate care and services
  • Ability to work with families of all ethnic and socioeconomic backgrounds.
  • Ability to work collegially and collaboratively with all disciplines.
  • Ability to work within boundaries of professional practice.

Nice To Haves

  • BSN preferred
  • Minimum 5 years pediatric nursing experience preferred.
  • Experience in working in clinical or community setting with children and families preferred.
  • Specialty licensure/certification may be required, as specified by department.

Responsibilities

  • Prioritizes, plans, provides direct family-centered care to patients within area of service throughout the continuum of care utilizing time and resources effectively and efficiently.
  • Facilitates the flow of patients within the clinical practice.
  • Partners with child and family in evaluating outcomes.
  • Monitors, trends and evaluates child/family responses to treatment throughout scope of service.
  • Provides appropriate education to patient/family regarding all aspects of care, including telephone advice, based upon established protocols.
  • Serves as advocate for parents and families.
  • Clearly documents all pertinent aspects of care, including: a plan of care; a regular summary of patient status and family adjustment; unexpected events of changes in patient condition; medications; and monitoring of vital signs, etc.
  • Complies with CCMC policies regarding appropriate documentation.
  • Adheres to current HIPAA Guideline to insure patient and family confidentiality is maintained.
  • Delegates aspects of care to peers and ancillary personnel to meet needs.
  • Serves as a resource to staff within area of expertise.
  • Networks with healthcare team by documenting and communicating comprehensive data including complex child/family needs to provide continuity.
  • Demonstrates knowledge of the age-related differences and needs of patients in appropriate, specific populations from neonate through adolescence and applies them to practice.
  • Demonstrates cultural sensitivity in all interactions with patients/families.
  • Values cultural diversity and other individual differences in the workforce.
  • Demonstrates support for the mission, values and goals of the organization through behaviors that are consistent with the CT Children’s standards.
  • Performs other related responsibilities as assigned.
  • Travel to satellite locations o support departmental demands as needed.
