About The Position

Scriptology is a tech-enabled comprehensive medication management (CMM) service augmented by an extended clinical pharmacy team. We help patients navigate their medication journey after leaving the clinic, offloading unnecessary burden from providers in a cost-effective way. We envision a world where the medication journey is humanized for patients at every step, and we are seeking a dedicated Clinical Care Coordinator to join our team. We are seeking a Clinical Care Coordinator for a phone-intensive, patient-facing role. The majority of this position involves outbound and inbound phone calls with patients to support medication management services, encourage medication adherence, and schedule clinical pharmacist consultations. This role is ideal for someone with prior healthcare experience who is comfortable on the phone, highly organized and who enjoys helping patients through clear, empathetic communication. You will work closely with pharmacists, providers, and internal clinical teams to ensure patients receive timely, coordinated care.

Requirements

  • 2+ years of healthcare experience required (e.g., Medical Assistant, Pharmacy Technician, Care Coordinator, Patient Scheduler, or similar)
  • Strong verbal communication skills and excellent phone etiquette
  • Comfortable spending the majority of the day on inbound and outbound calls
  • Familiarity with medical terminology and healthcare workflows
  • Experience using EHRs, telephony systems, and basic Microsoft tools
  • Ability to work independently in a remote, HIPAA-compliant environment
  • Reliable high-speed internet and a quiet, private workspace

Nice To Haves

  • Pharmacy Technician or Certified Medical Assistant (CMA)
  • Experience in ambulatory care, pharmacy services, or care coordination
  • Strong understanding of medications and common indications
  • Experience supporting quality measures (CMS Stars, HEDIS)

Responsibilities

  • Make high-volume outbound calls to patients to explain clinical pharmacy services and schedule pharmacist consultations
  • Handle inbound patient calls, answering questions and providing support related to appointments and medication services
  • Use structured conversations to encourage engagement and medication adherence
  • Build rapport and trust through calm, professional, and empathetic communication
  • Educate patients on the value of pharmacist-led medication management
  • Complete pre- and post-consultation tasks to ensure seamless care delivery
  • Follow all HIPAA and compliance requirements
  • Assist patients with logistics such as refills, savings programs, or appointment scheduling
  • Serve as a coordination point between patients, pharmacists, and provider offices
  • Document patient interactions and adherence-related notes in the EHR
  • Support quality initiatives such as CMS Stars and HEDIS measures
