Evaluates and assesses patients admitted to the Hospital. This role involves performing concurrent and retrospective reviews, determining appropriate referrals for alternate levels of care, and processing adverse determination decisions. The Clinical Care Coordinator also identifies problematic care patterns, participates in utilization management and discharge planning statistics, and stays updated on relevant policy changes. They attend committee meetings, participate in patient care rounds, and consult with physicians and healthcare professionals. A key aspect of the role is implementing and documenting discharge plans, coordinating with various hospital departments and external agencies, and arranging for post-hospital services. The position also involves identifying patient incidents through the NYPORTS program and communicating with physicians to clarify documentation justifying the quality of patient care. Additionally, the role requires staying informed about changes in Medicare/Medicaid/Commercial Insurances related to clinical documentation and utilization management processes.
