At Advanced Medical Home Care, we're looking for a licensed Physical Therapist Assistant (PTA) or Certified Occupational Therapy Assistant (COTA) to join our team as a Clinical Care Coordinator. This role allows you to continue improving patient outcomes while transitioning into a rewarding administrative position. In-office position Monday through Friday. As a Home Care Coordinator, you'll play an essential role behind the scenes, helping coordinate care and ensuring every patient has a smooth experience from start to finish. Requirements
- is a licensed COTA or PTA
- Has strong communication and organizational skills.
- Enjoys solving problems and managing details.
- Is compassionate, dependable, and patient-focused.
- Works well independently while thriving as part of a collaborative team.
- Current Minnesota PTA/COTA license in good standing.
- Knowledge of medical terminology
- Good judgment and decision-making skills.
- Strong attention to detail.
- Technologically proficient and driven by accuracy.
- Regular attendance and flexibility with scheduling necessary.
Responsibilities
- Coordinating communication between physicians, referral sources, patients, caregivers, and our clinical team.
- Reviewing and verifying medications while assisting with plan of care coordination.
- Supporting scheduling and administrative processes that keep patient care running efficiently.
- Working closely with clinicians to ensure timely, compassionate, and well-coordinated care.
- Helping create a positive experience for patients, families, and healthcare partners.
