Clinical Care Coordinator
About The Position
The Clinical Care Coordinator position will provide immediate linkage to appropriate levels of care for women identified through comprehensive assessment as requiring enhanced services. Responsibilities will include conducting comprehensive SUD /MH scales, using the state-funded vehicle to assist with transporting and ensuring access to outpatient treatment, recovery support services, community resources, and specialized PPW residential treatment programs, including state-funded inpatient PPW programs. Must possess valid NC driver’s license and be able to clear a motor vehicle check in order to transport minors.

Responsibilities
