Performs injury assessments and refers patients for necessary care. Communicates with patients to triage, answer questions, and instruct in self-care, prevention, and treatment. Collaborates with physicians and the medical team to determine the need for special tests or services. Establishes and implements a plan of care based on evaluation results, health goals, and appropriate techniques. Communicates the plan of care to all involved parties. Monitors and evaluates outcomes, adjusting the treatment plan as needed based on patient response. Recognizes potential issues and advocates for the patient by working with the physician and/or key professionals. Identifies and addresses factors affecting the patient's physical and psychosocial comfort. Able to work with a diverse patient population. Plans and completes patient after-visit summaries, including providing written home exercise programs, patient/family teaching, referral for community services, and ordering equipment. Completes all required documentation following department policy, including patient assessment, progress notes, and after-visit summaries. Performs various patient positioning, transferring, and transporting duties, which require lifting and pushing/pulling, while utilizing proper technique. Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. Must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his/her age-specific needs, and to provide the care needed as described in the department's policies and procedures.
