Performs injury assessments and refers patients for necessary care. Communicates with patients to triage, answer questions, and instruct in self-care, prevention, and treatment. Collaborates with physicians and the medical team to determine the need for special tests or services. Establishes a plan of care and implements individual treatments based on evaluation results, health goals, and appropriate techniques. Communicates the plan of care to all involved parties. Monitors and evaluates outcomes, adjusting the treatment plan based on patient response. Recognizes potential issues and advocates for the patient by working with the physician and/or key professionals. Identifies and addresses factors affecting the patient's physical and psychosocial comfort. Works with a diverse patient population. Completes patient after-visit summaries, including providing written home exercise programs, patient/family teaching, referrals for community services, and ordering equipment. Completes all required documentation according to department policy, including patient assessment, progress notes, and after-visit summaries. Performs patient positioning, transferring, and transporting duties, which require lifting and pushing/pulling with proper technique. Must demonstrate knowledge and skills to provide care appropriate to the age of the patients served, understanding principles of growth and development and assessing data reflective of patient status to identify age-specific needs.
