This role will support the Pennsylvania Clinical Assessment Team. The Clinical Assessor II provides a range of clinical services in accordance with program requirements, applicable regulations, accreditation standards, and organizational policies. Responsibilities include assessment, treatment planning, counseling, case management, crisis intervention, referral coordination, discharge planning, group facilitation, and other assigned clinical services. This role collaborates with clients, families, referral sources, community partners, and interdisciplinary treatment team members to support engagement, continuity of care, and positive treatment outcomes.

Requirements

  • Master’s Degree from an accredited college with a major in chemical dependency, psychology, social work, counseling with a Master’s level licensure: Marriage and family therapy, social work, professional counseling ), or certification by a state wide certification board (CAADC).
  • Master’s Degree from an accredited college with a major in chemical dependency, psychology, social work, counseling and are working towards a Master’s level licensure (Marriage and family therapy, social work, professional counseling), or certification by a state wide certification board (CAADC)
  • Bachelors level with a CADC or are working towards a CADC with a minimum of one year of qualifying clinical experience.
  • Associates level with a CAAC or working towards a CAAC with a minimum of two years of qualifying clinical experience.
  • High School Diploma or equivalent with a CAAC.
  • Current licensure in this Commonwealth as a registered nurse and a degree from an accredited school of nursing and one year of qualifying clinical experience.

  • Ability to gather, organize, and communicate assessment-related information accurately.
  • Ability to establish and maintain professional relationships with clients, families, referral sources, and community partners.
  • Ability to problem solve by gathering and analyzing information.
  • Ability to recognize crisis situations and communicate concerns appropriately.
  • Working knowledge of behavioral health, substance use disorder treatment, and recovery-oriented care.
  • Working knowledge of federal, state, accreditation, and compliance standards.
  • Ability to maintain confidentiality and professional boundaries.

Responsibilities

  • Provide clinical services, including individual, family, and group sessions, and maintain communication with referral sources and outpatient providers as needed.
  • Conduct mobile behavioral health and substance use disorder assessments within assigned service areas.
  • Complete biopsychosocial assessments, clinical evaluations, intakes, and reassessments.
  • Assess clients’ behavioral, emotional, cognitive, social, environmental, and recovery support needs.
  • Perform ASAM assessments and determine appropriate levels of care in compliance with credentials, licensing standards, and regulations.
  • Develop admission, treatment, and clinical recommendations based on assessment findings.
  • Complete individualized treatment plans in collaboration with clients and the treatment team.
  • Collaborate with referral sources, hospitals, emergency departments, providers, families, and community partners regarding treatment and admission planning.
  • Partner with treatment teams to support service planning and clinical care needs.
  • Facilitate utilization review processes and maintain documentation to support authorizations and continued stay reviews.
  • Support aftercare planning, referrals, and continuity of care coordination.
  • Maintain timely, accurate, and compliant documentation in accordance with regulatory and organizational standards.
  • Participate in on-call rotations as assigned.
  • Maintain confidentiality and exercise appropriate professional discretion.
  • Complete all required trainings per organizational, licensing, and accreditation requirements.
  • Perform other duties as assigned.
