About The Position

The Clinical Assessment and Eligibility (CAE) Registered Nurse (RN) at Boston Senior Home Care functions as part of an interdisciplinary team and participates in the assessment of consumers in both community and nursing facility settings. Responsibilities include conducting comprehensive clinical assessments to determine eligibility for programs funded by the Executive Office of Aging and Independence (AGE) and the Division of Medical Assistance, as well as evaluating clinical eligibility for nursing facility level of care.

Requirements

  • Valid Registered Nurse's license from the Commonwealth of Massachusetts
  • Valid driver's license
  • Bachelor's degree in Nursing or an Associates of Science Degree in Nursing and at least one (1) year of clinical experience and/or nursing in a community or long-term care setting; or an Associate Degree in Nursing and five years of experience, with two years of community-based nursing or long-term care setting.
  • Diploma RN and two (2) additional years of community-based nursing experience or long-term care direct service experience.
  • Knowledge of the mental health, elderly and developmentally disabled populations.
  • Strong assessment, independent caseload management, and interpersonal skills.
  • Ability to communicate effectively and work collaboratively with consumers and co-workers.
  • Solid computer skills and experience.
  • Must be able to work in the United States

Nice To Haves

  • This person must be supervised by the Nurse Manager, or A Diploma RN and two (2) additional years of community-based nursing experience or long-term care direct service experience. This person must be supervised by the Nurse Manager.

Responsibilities

  • Conducts nursing assessments for MassHealth or MassHealth pending consumers, who reside in the community, for Home Care programs and/or for nursing facility admission; determines clinical eligibility for those consumers in nursing facilities.
  • Participates in the peer review process of consumer records.
  • Provides consultation to case managers and other staff members regarding medical and nursing issues, insurance reimbursement and the Money Follows the Person Demonstration, MFP.
  • Through the Section Q process, identifies potential MFP consumers and other nursing facility consumers who wish to return to the community.
  • Conducts weekly nursing facility onsite visits, meets with facility social services and conducts clinical eligibility assessments.
  • Takes part in interdisciplinary team meetings in nursing facility settings to support a consumer's timely transition to the community and enrollment in specialty Home Care and MFP demonstration programs and waivers.
  • Collaborates with BSHC and other ASAP registered nurses and case managers for Home Care consumers who presently reside in nursing facilities.
  • Performs clinical assessments and screenings for consumers in the BSHC area for programs and services funded by DMA.
  • Provides support and training in the screening process to hospitals, nursing facilities, ADH and certified home health agencies.
  • Develops and implements in-service programs on medical issues for staff and outside agencies.
  • Maintains collaborative relationships with agencies and institutions that serve BSHC clients, and maintains cooperative and effective working relationships with internal and external customers.
  • Participates and assists with outreach programs.
