About The Position

PACE North is a Program of All Inclusive Care for the Elderly (PACE) serving individuals 55 and older who qualify for nursing facility level of care but wish to remain in their homes. Established in 2019, PACE North is a growing organization providing daily care at its day center and in participants' homes. Requirements

At least 2 years of experience in a healthcare setting.
1 year of experience working with frail or elderly populations.
Strong understanding of healthcare terminology.
Familiarity with integrated care models, such as PACE.
Excellent organizational and multi-tasking abilities.
Effective judgment for decision-making.
Proficiency in business software applications.
Strong communication and negotiation skills.

Nice To Haves

Post-high school education in a health-related field.
Familiarity with integrated care models, such as PACE.

Responsibilities

Ensures healthcare services are delivered in accordance with regulatory standards and organizational policies.
Coordinates and integrates services across an interdisciplinary team.
Maintains accurate and timely documentation.
Supports the creation and adjustment of individualized care plans.
Manages participant grievances.
Facilitates team meetings.
Identifies opportunities for process improvements.
Ensures adherence to compliance regulations.
Fosters effective communication among team members.
Plays a critical role in enhancing care quality and operational efficiency.
Supports reception duties daily.
