Voyage Healthcare is seeking a Clinical Support/Triage RN to provide patient-centered nursing care in an ambulatory clinic setting. This role includes telephone and in-person triage, nurse visits, immunizations, patient education, care coordination, and clinical support. The RN collaborates with providers and care teams to ensure safe, timely, and effective patient care while practicing within the Minnesota Nurse Practice Act and established clinical protocols.

Requirements

Graduate of an accredited school of nursing program
Current Minnesota Registered Nurse (RN) license
Current CPR certification
Strong communication, assessment, and critical-thinking skills
Ability to work independently and collaboratively in a fast-paced clinic environment

Nice To Haves

Two years of RN experience preferred

Responsibilities

Provide telephone and in-person triage using established protocols
Assess patient symptoms and determine appropriate level of care
Administer medications, injections, and immunizations
Conduct nurse visits and provide patient education
Assist with clinical procedures and treatments
Coordinate patient care and support case management activities
Process prescription refills according to protocol
Respond appropriately to urgent and emergent patient situations
Document patient assessments, interventions, and communications in the EMR
Deliver culturally competent, patient-focused care
Maintain compliance with organizational policies, OSHA standards and professional nursing practices
