About The Position

The Community Care Network LVN provides ongoing support and education to assigned patients under the direction and supervision of a Registered Nurse. Care is delivered following a plan established by the physician and the Care Management Director, in conjunction with the patient/family and in compliance with the California Vocational Nurse Act. The LVN works within the framework of the Community Care Network team. Directly assists patients and practitioners to navigate the health care system at an optimal level, supports care management activities and ensures patients receive the right services at the right level of care.

Requirements

  • Current CA Licensed Vocational Nurse (LVN) license is required.
  • Current Basic Life Support (BLS) Certification required.
  • Must have good communication skills and be able to organize and prioritize tasks effectively.
  • Ability to work independently and as a team player who builds effective working relationships.
  • Problem-solving skills, proactive thinking and ability to anticipate issues.
  • Ability to collaborate with clinical staff to address ongoing needs of patients.
  • Ability to understand psychosocial barriers and patient’s needs.
  • Good understanding of community resources.
  • Organizational and time management skills.
  • Verbal and written communication skills.

Nice To Haves

  • Certification in geriatric nursing or geriatric care management preferred.
  • Minimum of 5 years acute care LVN or related/equivalent experience is preferred.
  • Relevant experience in Care Management.
