About The Position

The Clinical Social Worker I/II (CSW) provides comprehensive psychosocial assessment, crisis intervention and 5150 services, counseling, brief therapeutic interventions, consultation, education, outreach, community linkages, and facilitates the plan of care for patients with particular focus on high-risk populations. Additionally the incumbent provides mental status examinations and diagnoses using the Diagnostic Statistical Manual (DSM5-TR); consultation to a multi-disciplinary treatment team; acts as liaison to hospital and community resources; maintains client records according to California State Medi-Cal and San Francisco City and County requirements; record data as required by Department of Public Health, attends staff meetings and participates in training sessions and performs other duties as assigned. Under the general supervision of the Clinical Supervisor, the Clinical Social Worker I/II works within the innovative Citywide Stabilization Program, which utilizes a proactive, client-centered approach to engage and treat adults with severe mental illness who are often dually diagnosed and unhoused. This program will be responsive to the needs of the Mental Health San Francisco (MHSF) initiative and will be fluid in service delivery based on the needs of the Behavioral Health Services under the Department of Public Health. The CSW I/II works collaboratively with the behavioral health system of care to develop creative treatment strategies. The incumbents job duties may include but not limited to below: Intensive Case Management: The incumbent carries a caseload of high risk and vulnerable patients that are struggling to engage with necessary mental health care. The service delivery will include low threshold intensive case management, stabilization, and linkage to the appropriate level of care. CARE Court: This position works with clients involved in the Citywide CARE Court program, referred by the Department of Public Health CARE Court Program, and provides intensive case management services to clients for the duration of their time in CARE Court. Assisted Outpatient Treatment (AOT)- Provides intensive case management to adults that are either court ordered to treatment or voluntarily agreed to treatment after meeting the legal threshold for AOT. These individuals require specialized treatment engagement.

Responsibilities

  • Provides ongoing mental health services, including brief psychotherapy interventions (Individual, Group, & Family), Crisis Intervention, referrals to case management, residential treatment and temporary housing or shelter.
  • Provides outreach and advocacy for clients wherever they are, including the streets, hospitals, jails, shelters, housing, and programs.
  • Develops a comprehensive individual service plan for each client, integrating knowledge of a wide range of human service resources.
  • Provides consultation to a multi-disciplinary treatment team on the psychosocial ramifications of psychiatric illness.
  • Facilitates treatment coordination among various agencies and advocates for and links clients to needed services.
  • Provides consultation to and maintains liaison with campus and community programs, including educational, legal, and social organizations involved in case management.
  • Acts as liaison to inpatient units to facilitate referrals and service treatment.
  • Assists with intakes of new clients, including assessment, community functioning, and service needs, when appropriate. This entails assessment of presenting problems, psychosocial history, mental status and diagnosis (DSM-V).
  • Maintains client’s records according to California, City and County requirements.
  • Records data in client’s charts.
  • Meets program productivity standards.
  • Assists in processing quality improvement requests.
  • Completes billing and other documentation of services in a timely fashion.
  • Attends staff and clinical team meetings, and participates in training sessions (e.g., In services, grand rounds).
  • Identifies program problems and makes recommendations to supervisory and administrative staff.
  • Provides education to the community regarding program services, client needs and treatment.
  • Assists in planning and providing in-service to staff.
  • Provides orientation and didactic training to interns.
  • If desired and appropriate, supervises student(s) and participates in student training program meetings.
  • Performs other duties as assigned.
