About The Position

The Client Support & Service Coordinator for Hope Manor Village Joliet (HMVJ) provides trauma informed case management and supportive services to residents in VOA Illinois’ recovery focused supportive housing community. This role supports women and families in maintaining housing stability, progressing in recovery, and achieving long term self sufficiency. Responsibilities include coordinating services, developing individualized service plans, and helping prevent housing crises through financial assistance support such as rent or utility aid and connections to internal and community resources.

Requirements

  • Bachelor’s degree in Human Services, Social Work, Psychology, Counseling, or related field
  • 2+ years of experience in case management, supportive housing, behavioral health, or working with vulnerable populations
  • Experience providing trauma-informed care and connecting clients to community resources
  • Strong communication, collaboration, and conflict de-escalation skills
  • Proficiency in Microsoft Office and electronic documentation systems
  • Community-based travel required
  • Valid driver’s license required
  • Ability to lift up to 50 lbs.
  • Sedentary work, computer use, and frequent communication with clients, colleagues, and partners involved in client support.
  • Must be able to travel within the community, transport clients when necessary, and assist clients in and out of vehicles

Responsibilities

  • Provide intensive case management and crisis intervention for residents recovering from opioid addiction
  • Conduct client intake and comprehensive assessments to evaluate psychological, physical, recovery, and service-related needs
  • Develop and monitor individualized service plans that support housing stability, recovery goals, parenting needs, and overall well-being
  • Conduct monthly home visits and community-based meetings to monitor client progress and service outcomes
  • Support eviction prevention by helping residents resolve housing crises through financial assistance coordination (rent or utility aid) and connections to internal and community resources
  • Assist residents with program applications, documentation, and eligibility requirements for supportive services and financial assistance
  • Advocate for residents and connect them with treatment providers, healthcare, parenting resources, employment supports, and community services
  • Serve as a liaison between residents, property management, and community partners to coordinate services and promote housing stability
  • Educate residents individually or in groups on available services, recovery supports, and tenant rights and responsibilities
  • Utilize trauma-informed care, motivational interviewing, and conflict de-escalation strategies when supporting residents
  • Maintain accurate documentation in EchoVantage, HMIS, and other required databases while managing assigned caseloads and ensuring program compliance
  • Promote a recovery-focused and family-centered community environment that supports healing, stability, and long-term success
  • Collaborate with VOA Illinois staff, vendors, and community partners to ensure coordinated service delivery
  • Exhibit cultural competence while serving residents of diverse backgrounds and commit to expanding knowledge and understanding of different communities and people groups
