About The Position

HealthRIGHT 360 is a nonprofit organization dedicated to providing accessible and comprehensive healthcare services to vulnerable populations. Our mission is to address systemic barriers to healthcare and promote health equity. We offer a range of services including mental health care, substance use treatment, and primary health services, along with transitional support for individuals re-entering the community from the criminal justice system. By integrating physical and behavioral health, we aim to empower individuals to overcome challenges by addressing social determinants of health, fostering resilience, and facilitating recovery. Prototypes Pomona Recovery Bridge Housing (RBH), a program of HealthRIGHT 360, provides interim housing for unhoused adult and adult perinatal women receiving outpatient treatment. RBH offers recovery-oriented services such as peer support, group meetings, case management, and life skills development, aiming to stabilize residents' lives and prevent relapse through safe, supportive housing. Residents, prioritized by vulnerability, can stay for up to 6 months annually in 3-month increments and must remain engaged in outpatient treatment. The Client Safety Navigator (CSN) is responsible for fostering a safe, respectful, and welcoming environment. The CSN collaborates with program leadership, law enforcement, and other teams to respond to emergencies and crisis situations, including client drug use, overdose, and programmatic interruptions.
